Executive wants to frame the NIH indirects cut as $4B in savings.

Despite evidence supporting the cost-effectiveness of polypills, no manufacturer has invested the millions of $$$ required to gain FDA approval. Hopefully, this study will signal demand for the cardiovascular polypill to policymakers and manufacturers.

For all trial-eligible non-Hispanic Black adults, polypill treatment was high value ($13,400-per-QALY gained).

It also yielded more QALYs and lower ICERs among poorer individuals, suggesting the polypill could cost-effectively reduce income-related health inequalities.

Findings!

For a trial-representative cohort of 100,000 individuals, compared to usual care, polypill treatment:
- prevented 2,738 CVD events
- yielded 1,190 QALYs
- increased costs by ~$10,152,000

Hence, polypill treatment cost $8,560-per-QALY gained and was high value.

In the base case, we assumed an annual polypill price of $463: the combined median price of 3 half-standard dose BP-reducing medications and intermediate-intensity statin therapy.

Participants were randomized to receive a polypill (single pill containing a statin and 3 half-standard dose antihypertensive meds) or usual care.

After 1 year, the polypill group experienced greater SBP (7 mmHg) and LDL cholesterol (11 mg/dL) reductions than those in the usual care group.
