Yep. I’ve also seen it a fair few times with spiro - on occasions <125 mmol/L.
Paired osmolalities in a couple of stable pts w mild hyponatraemia on spiro suggest it’s due to increased ADH.
Our research recommendations ….
Should we have included an RCT or intensive vs. relaxed BP goal in dialysis patients?
If yes, what BP measurement and what BP goals? What about patients on PD?
BID study provides some insights into how challenging this could be to deliver!
Our @ukkidney.org guidelines here in BMC Nephrology. 💯% agree that in-unit BP should ONLY be used for determining safety of individual HD sessions and NOT long-term CV risk modification. But changing practice will be a long haul.

Proud to have worked with colleagues and friends around the world on #ACHIEVE. We're disappointed spiro didn't prevent CV death/heart failure hospitalizations, but we've shown it's possible to do publicly funded large RCTs of repurposed meds in ESKD. More to do!
