The hospital doesn't blow up on day 1 but it's still slowly bleeding out due to loss of productivity when you're faced with the reality of trying to patch on what you should have built from the start but with a much smaller team.

Why does that matter? The EHR doesn't do what the hospital needs it to. There's no consideration for user or business needs, just delivering a minimal build that worked last time and so there's no headlines when the switch gets flicked.
