Our new paper on vancomycin AUC dosing in obesity is out. It showed decreased AKI in obese patients using AUC dosing as compared to trough dosing. Rates of AKI were lower in the AUC group compared to the trough group (11.3% versus 25.2%, p<0.001). This work was led by Corey Guidry .

Yeah, we dose adjust up all the time. Obese patients do worse (pubmed.ncbi.nlm.nih.gov/32062851/) with candidemia. Obese patients have lower exposures than normal weight patients (pubmed.ncbi.nlm.nih.gov/37272474/). There hasn’t been data that I’m aware of showing improved outcomes with higher doses

If valve cultures are negative, then I’d go by clearance of blood cultures. If valve cultures are positive, I’d restart the clock.
