I spent an entire hour the other day in clinic discussing, making med adjustments, and educating a patient with some complicated chronic diseases. It was a great conversation, and the patient left feeling confident with the new plan. But I recognize that time is a luxury in most settings. #PharmSky

I was gonna say that too, but but I ran out of room! š Many of our patients are on metformin ER 500 mg 2 tabs BID because insurance wonāt cover the 1000 mg tabs.
Medication only works if it actually makes it into your body. Iāve had patients tell me they were allergic to metformin and couldnāt take it because it gave them diarrhea. #PatientEducation #Diabetes
Pharmacy Pearl of the Day (PPotD): Even if your patient needs 2000 mg a day of metformin, donāt start there. Do 500 mg HS x 1 week, then 500 mg BID. Then 500 mg QAM + 1000 mg HS, then 1000 mg BID. Let their gut adjust, because if the GI effects are too much, theyāre not gonna take it. #PharmSky

Taking it daily increases your bodyās production of hepcidin, which blocks iron absorption. So ever other day dosing gives you the most effectiveness (and fewer GI side effects as an added bonus)

If youāre taking iron supplements, youāll get best results if you take it every other day. Even better if you wash it down with orange juice. (The vitamin C helps your body absorb the iron.)
So technically rifamycins (like rifampin) are proven to affect OCs, but anecdotally, there are case reports (and a couple of friend instances) of OC failure with ABX. In the current political climate, Iād want to be extra careful just in case š¬

Great question! Right now it depends on what state youāre in. We can bill incident to in some places, and can act as an agent for MD billing for annual wellness, transition of care, and chronic care management. If only we could get CMS on boardā¦
Just my opinion, but if you included a clinical pharmacist to manage chronic medication management (and deal with pharmacy communication and patient med access), it would be a step in the right direction. Weāre qualified and ready to help.

Absolutely! These are all things I get tasked with, so itās great that other prescribers will see this. But Iād love to connect these authors with some independent pharmacy owners to talk about what all they can offer.
Iām so glad clinicians are working on improving access to medications, but Iām curious why there are no pharmacists included as an author. I feel like they could have provided a valuable perspective. #PharmSky
My undergrad degree is in music and theatre, and Iām still actively involved in the arts. I draw on those skills every day as a clinical pharmacist.
