Internet Addiction Among School Children: Cross-Sectional Analytical Study Background: Internet use is rapidly increasing in Sri Lanka. Excessive use can lead to addiction with significant consequences, particularly among adolescents. While internet addiction has been documented worldwide, data from Sri Lanka remain limited. A validated local tool is required to assess the prevalence and associated factors in this population. Objective: This study aimed to translate and validate the Young Internet Addiction Test (IAT) into Sinhala, assess the prevalence of internet addiction among school-going adolescents aged 15 to 19 years in the Western Province of Sri Lanka, and identify demographic and behavioral characteristics associated with internet addiction. Methods: We conducted a 2-phase cross-sectional analytical study in Colombo and Gampaha districts. Phase 1 involved translation and validation of the Sinhala IAT using confirmatory factor analysis (n=200) and test-retest reliability assessment (n=40). Phase 2 involved multistage stratified cluster sampling to recruit 2835 students. Participants completed self-administered questionnaires assessing demographics, internet use patterns, and internet addiction. #feasibility and Engagement of a Peer-Driven Mobile Intervention for Adolescent E-Cigarette Cessation: Cluster Randomized Pilot Study Background: E-cigarette use remains prevalent among US adolescents, with many reporting daily use and high nicotine dependence. Few evidence-based mobile health interventions focus specifically on adolescents. Objective: This study aimed to evaluate the #feasibility, engagement, and preliminary efficacy of vaper-to-vaper (V2V)—a multicomponent, peer-driven texting intervention supporting adolescent e-cigarette cessation. Methods: A cluster randomized pilot study was conducted in 5 Massachusetts high schools, with schools randomized to either the V2V texting intervention (n=3) or a control group (n=2) that received a link to the National #Cancer Institute’s Smokefree.gov Quit Vaping website. The V2V intervention included four components: (1) peer-written messages provided motivation, tips, and strategies to support adolescents in quitting vaping, sent daily in the first 30 days; (2) peer videos featuring adolescents sharing their experiences with e-cigarettes and motivations to quit, sent regularly as links aligned with related peer message topics; (3) peer coaches—university students aged younger than 22 years who had successfully quit vaping—trained to provide support, encouragement and answers to participants’ questions through the texting platform; and (4) a fictional, gamified mystery story integrated into the texting platform to promote engagement. Each gamified message included a short story segment and a question, with the next segment unlocked after a response or automatically after 3 days. The intervention was mainly delivered over 30 days, but adolescents could message the peer coach over the 3 months. Eligible participants (grades 9‐12, current e-cigarette users) were followed for 3 months. We assessed the #feasibility of recruitment and retention (target: 80 participants, ≥85% retention), engagement with intervention components, and participant satisfaction. The secondary outcomes included improvements from baseline in confidence to quit, self-efficacy to resist vaping in specific high-risk situations, and fewer days vaped. E-cigarette cessation was biochemically verified using the Abbott iScreen cotinine test. Results: Seventy-one adolescents enrolled (intervention: 39/71, 55% ; control: 32/71, 45%), with a 96% follow-up rate at 3 months. Among intervention participants who responded to engagement items (N=37), high engagement—defined as self-reported use always, usually, or about half the time—was highest for peer messaging (n=29, 78%), followed by gamification (n=18, 49%), peer coaching (n=18, 49%), and peer video (n=13, 35%). The intervention group showed nonsignificant improvements in confidence to quit (n=17, 46%, vs n=9, 24%, moved from not at all, somewhat, or moderately confident to very or extremely confident) and in the number of days vaped in the past 30 days (−3.6 vs −2.9), while self-efficacy scores (adapted smoking self-efficacy scale range 12‐60) were slightly lower compared to the control group (mean −0.21, SD 1.14, vs mean 0.06, SD 1.39). Cotinine-validated 7-day point prevalence abstinence was similar between groups (intervention: 21.6% vs control: 22.6%). Conclusions: The V2V intervention demonstrated #feasibility and acceptability, with strong engagement and high satisfaction. Although differences between groups were not statistically significant, findings suggest that peer-driven mobile interventions are a promising approach to support adolescent e-cigarette cessation. School and Community Stakeholder Perceptions of a Free, Confidential Digital Mental Health Platform (Soluna): Mixed Methods Study Examining Barriers and Facilitators to Real-World Implementation at Scale and Early Impact Background: Mental health disorders are common among adolescents worldwide; yet, access to preventive and early intervention services remains limited. Digital mental health platforms may help bridge this gap, but little is known about how these platforms are perceived, implemented, and adopted by school and community stakeholders during early stages of rollout in real-world youth-serving settings. Objective: This study aimed to examine school and community stakeholders’ perceptions of a new, free, confidential digital mental health platform for youth (Soluna by Kooth Digital Health) in its first year of rollout, focusing on its features, barriers, and facilitators to early implementation and adoption, and early stakeholder perceptions of its perceived impact on youth mental health. Methods: Surveys were distributed to 77 stakeholders (54 school staff and 23 community staff) in California from February to April 2025. Eligible participants were frontline staff directly engaging with youth in settings where the platform was offered. Following the survey, 17 stakeholders (12 school and 5 community) participated in semistructured interviews via Zoom (Zoom Communications). Survey data were analyzed descriptively to summarize perceptions and experiences, while interview data were analyzed using inductive-deductive reflexive thematic analysis to explore themes related to implementation, facilitators, and barriers. Results: Most stakeholders (71.4%) agreed that the platform positively contributed to youth well-being, and 89.6% felt comfortable referring youth to it, indicating good acceptability. Free access (91%), availability during nontraditional hours (45%), and ease of use (42%) were identified by stakeholders as the most valuable features for youth. Facilitators of adoption included digital accessibility (87%), confidential peer support (84.1%), and youth choice in engagement (85.7%). Key barriers included stigma around mental health (64.7%) and lack of awareness of the platform’s benefits (61%). Qualitative findings revealed three main categories: (1) perceived impact, including support for youth from diverse backgrounds and use as a supplemental resource when traditional services are limited; (2) facilitators to implementation and adoption, such as direct referrals, peer promotion, accessible framing focused on coping and life skills, supportive materials, and engagement from platform staff; and (3) barriers to implementation and adoption, including stigma and phone or internet access challenges in school settings. Stakeholders also emphasized the importance of practical resources and ongoing support to build confidence in using and recommending the platform. Conclusions: Stakeholders viewed the platform as a valuable and accessible tool to support youth mental health, particularly in underserved communities. Findings suggest that digital mental health platforms should address external barriers to adoption, including stigma around mental health, while also increasing awareness of available resources. Providing tailored implementation support demonstrates progress in these areas and can further strengthen adoption and engagement. These findings offer actionable recommendations for improving the design and delivery of digital mental health platforms in real-world youth-serving settings.

Exploring Strategies for a Digital Tool to Support Medication Adherence Among Adolescents and Young Adults Undergoing Hematopoietic Stem Cell Transplant and Their Care Partners: Qualitative Formative Study Background: Allogeneic hematopoietic stem cell transplant (HCT) is a complex but essential treatment for malignant and nonmalignant conditions, requiring strict posttransplant adherence to immunosuppressant medications to prevent complications such as graft-versus-host disease. Adolescents and young adults undergoing HCT face unique challenges, including balancing growing independence with ongoing reliance on care partners, often parents. Medication adherence in this group is often suboptimal, and few interventions address adolescent and young adult–care partner dyads. To address this gap, we aim to develop a mobile health (mHealth) app that engages both the patients and care partner to improve adherence. Objective: As formative research for early-stage intervention development, this study aimed to (1) explore current HCT medication adherence strategies and challenges; (2) understand attitudes toward digital technology, including dyadic perspectives on app use to support adherence; and (3) assess adolescent and young adult–care partner relationships, including views on care partner involvement. This process was intended to inform the design of a relevant, user-centered mHealth app. Methods: Eligible participants included adolescents and young adult patients aged 12-39 years and primary care partners, such as parents, involved in medication management. Participants were recruited from a large academic medical center through direct outreach and electronic health records. Data collection involved 2 focus groups (6 dyads and 2 additional adolescents and young adults), 4 individual interviews (2 patients and 2 care partners), and 6 dyadic interviews. Semistructured sessions (in person or virtual) gathered feedback on medication adherence practices and app design preferences. All sessions were audio recorded with consent and professionally transcribed. Qualitative data were analyzed systematically: transcripts were deidentified, coded using both inductive and deductive strategies, and themes were refined through team consensus. Patterns were organized into major themes, and representative quotations were selected to illustrate findings. Data management was facilitated by NVivo (version 13; Lumivero) software. Results: We included 28 participants (15 adolescents and young adults and 13 care partners). The median age of adolescents and young adults was 18 (range 13-39) years and 53% (8/15) were female. Adolescents and young adults were 47% (7/15) White, 40% (6/15) Black, and 13% (2/15) mixed race. Care partners’ median age was 48 (range 36-72) years, with 92% (12/13) female and 77% (10/13) White. Three principal themes emerged: (1) existing reminders and organizational tools are often insufficient for consistent adherence; (2) adherence barriers are multifaceted, often involving autonomy vs care partner support; and (3) both adolescents and young adults and care partners showed strong interest in a dyadic digital health intervention to foster collaboration and support shared adherence goals. Conclusions: This formative study highlights the complex dynamics of medication adherence in adolescent and young adult–care partner dyads and supports the need for a dyadic mHealth app to enhance adherence, collaboration, and relationship quality.
