Studied how supportive communities help people with disabilities stay independent. Talked with nearly 200 people to find out what kind of support families need. Created information on moving to a new home and planning for the long term. Made tools in Spanish so more families can share what they need.

They have: - Added a way for people to make their own choices into state care plans - Started a website to show families other ways to make choices besides guardianship - Added new tips for the future to state planning forms - Taught workers how to help both the person with a disability and their family
