A white side which reads: Top tips from case 5 - complexity is our bread and butter - embrace the chaos of not knowing - invest time in effective communication - it'll make you feel more confusing with decisions - don't go into all conversations with a pre-set agenda - flexible (but clear) decide making during the communication is ok - team work is great but be cautious of the non-invested opinion - 'he definitely isn't a candidate for escalation.' 'I'd put him on NIV.'

Embrace the chaos! Great top tips for complex patients from Dr Nick Saxton at @gerisoc.bsky.social #G4J #bgsconf.
Dr Henry Woodford speaking at a lectern. The lectern has the BGS logo on a pink background on the front.
Great to have stayed on in Nottingham for the @gerisoc.bsky.social #G4J #bgsconf. Fab to see so many people here and great talks. Really enjoying this one on polypharmacy from @henriatric.bsky.social (who also has a great bluesky handle).
On way back from #bgsconf in Nottingham and driving conditions so treacherous I had to bail at Leicester East Days Inn. Scary stuff.
Luxury for £35 a night. But better than being in a life changing accident..

Mary Harrington presents about skin care at end of life. Mary supports police investigations to identify abuse as opposed to normal care. Previously in this session Gary Jevons presented evidence of bruising for accidental & non-accidental #BGSconf @gerisoc wearehourglass.org/who-we-are

Prof Dhesi gives a presidential opening speech at the #BGSConf
Good morning everyone.
I’m at the #BGSconf this morning talking about delirium.
Caught @cswarbrick1’s excellent talk on the impact of delirium and the results of Snap3.
Delirium increases length of stay, mortality and all complications.
Sorry everyone at the QI strand of the #BGSconf trainees' meeting for my sudden outburst of Trent Crimm fangirling 🫣
Very powerful speech from Dr Jennifer Burns about being BGS president during COVID and the lesson learned.
Especially appreciated her answer at the end calling for the need to improve how we train the doctors of the future and reduce the rigidity of current training.

Helpful update from Dr Glynis Magee providing an update on managing diabetes in the frailer patient.
Useful guidance on target HBA1C and prescribing recommendations- including Target HBA1C of up to 79 in severely frail patient.
Main emphasis is on individualised plan for patients
#BGSconf
Which raises the interesting discussion between geriatric medicine Vs care of the elderly.
I personally prefer the former as emphasises it as a medical specialty skillset.
While care of elderly does sound a tad patronising.
Plus CoE just makes me ready to debate transubstantiation.
#BGSconf
As part of the discussion of Ageism, depressing statistics but also interesting points from Adam Stachura of Age Scotland.
Question of the use of elderly for our patients and carrying negative implications.
#BGSconf
Mainly want to catch on the dementia update from yesterday.
Especially the question of geriatricians being involved in the diagnosis of dementia in hopsital.
Feels like a missed opportunity to send patients to memory clinics in certain cases. (Although understand logic of it!)
#BGSconf
Back for BGS spring meeting today (optimism to watch day 2 while looking after toddler was a tad naive)
Catching up on things I missed such as the Presidents address- including revaluation that BGS is biggest medical specialty association in the UK!

Sarah Combes
@sarahcombs.bsky.social
presents advance care planning challenges in the words of patients.
Key takeaways:
FAMILY- most older people know what they want but don’t tell their family
START with living well now and work the conversation backwards.

How do we bridge the workforce gaps in delivering future care for an ageing population, closer to home?
@gerisoc.bsky.social Rising star for Research @abijhall.bsky.social tells #BGSconf how #AHPs and support workers are key to delivering this— AHP community #Research 🔑🔑
Not sure if it's offered in our area or if it's still a tad experiential (watching in delay so missed a chance to ask a question)
#bgsconf
Quite informative talk on chronic SDH management, especially on physiology of profession and rexurewnce.
Discussion about the benefits of patients having surgery even with less frank neurology.
And the concept of middle meningeal artery embolization as an alternative 1st line treatment.
