Functional MR is a diagnosis of exclusion. Don't call it "functional" without confirming leaflet mechanism.
1️⃣ Leaflets: Look for flail or prolapse. If broken, it’s Organic.
2️⃣ LV: If leaflets are intact but heart is dilated, it's Functional.

Excessive Doppler gain amplifies noise, overestimating velocities.
Adjust gain until the background is black and the spectral window is clear. Signals should be thin and well-defined; over-gaining falsely broadens the trace and increases measured velocity.
