Learning from experience in perioperative allergy: how the patient voice should shape clinical care Understanding the experiences of individuals using the health system is crucial to be able to explain future treatments and address concerns. This is …

Proud of this new editorial out today in @bjajournals.bsky.social
Mostly because it’s an innovative way of writing, including the patient as the primary author and hearing their story.
Also because it touches on human centred design.

Hey #AnSky #HFESky - a weekend topic for you:

a) What have been the greatest Human Factors / Ergonomics advances in anaesthetics in the last 100 years?

b) What, if anything, has anaesthetics contributed to the wider field of HF/E?

I’m amazed by this from a safety perspective.
There’s a thing called the Peltzman effect - deliberately accepting more risk because safety measures have been introduced.
Peltzman found people engaged in riskier behaviour and had more crashes when car seatbelts were introduced. #hfesky #safetysky

Ep. 126: Is it time to stop talking about safety culture? - The Safety of Work Do you know the science behind what works and doesn’t work when it comes to keeping people safe in your organisation? Each week join Dr Drew Rae and Dr David Provan from the Safety Science Innovation...

Excellent discussion about a paper that implores us to stop researching and talking about 'safety culture'. It is a term that has stopped being useful and it is sucking time and energy away from things that can actually be effectively researched and improved.

… and I say this as a white cis-het man whose pathway through the system is charmed.
We need to think intersectionally about how others are treated.
We can do better.

Learning from Everyday Work - Part 1 A series on Learning and Patient Safety

I don't think Satyan is over here....yet 😉 but his Human Stream newsletter is a fantastic resource. I'm fortunate enough to get to chat with him about this stuff IRL but each issue is a great conversation starter.

Delighted to be here in Stavanger to speak at the 18th International Behavioural Science Applied to Healthcare conference.

Starting the day listening to Guttorm Brattebo from University of Bergen talking about teamwork in emergency settings.

Including everyone in designing hospitals and systems, getting their viewpoints and undertaking iterative design helps navigation of hospital systems and improves outcomes.

Systems are complex and exploring viewpoints essential. Listening to the marginalised often gives surprising insights. Designing for the marginalised often leads to benefits for everyone. A phenomenon called the "Curb cut effect"

I'll be talking at the Periop Medicine special interest group / Summit III and POQI meeting in Melbourne very shortly.
How do we use Human centred design to make it easier for patients and relatives to prepare for and recover from surgery?

Being a 'good rescuer' is:

• counterintuitive eg the advice to stand back & analyse rather than pile in & do something
• counter-cultural eg using checklists rather than relying on memory

Specific rescuer training would reinforce these skills.

What is a common thread in medical tragedies?

Rescuers fail to cut through the ‘mind lock’; they follow the primary caregiver down the wrong, fatal, path.

If we are going to train our way out of this problem, we need to train the rescuers.

Why do skilled practitioners make errors?

Mind lock - a rigid fixation on a diagnosis/course of action

Confirmation bias - contradictory data (eg abnormal capnography) are ignored, while corroborative information (eg breath sounds) is favoured

It’s likely that experts gain less in terms of reduction of physical stress due to awkward body posture than novices as they have refined technique over time.

Maybe though, training of DL & VL both need to emphasise body posture and physical ergonomics of intubation?

Body posture during simulated tracheal intubation: GlideScope® videolaryngoscopy vs Macintosh direct laryngoscopy for novices and experts Tracheal intubation requires the anaesthetist to adopt an awkward body posture. To investigate how posture may be improved, we compared the effects of laryngoscopy technique (GlideScope® vs Macintosh...

There’s definitely a difference in posture between novices and experts. Tobi Grundgeiger and his team noted this:

It could be that experts benefit in different areas like leg and neck flexion?

Ding et al argued a benefit of #AugmentedReality laryngoscopy was improved intubation ergonomics.

Inoue et al state "it may not be accurate to claim that all users adopt a forward-leaning posture".

Your thoughts?

This speaks to trust and the degree of automation. A good analogy is that of a self driving car. A self driving car and a purely manual car might be safer than partially autonomous vehicles as the purpose of the automation and trust in it is unclear
Other #HFESky folks know more than I do on this!

There are two types of anaesthetist in this world:

Those who think there are two types of anaesthetist, and those who don't.

I don't.

Chew on that.

I'll select this one as it covers a lot of work in Human Factors over the last few years and cites the excellent DAS/Association guidelines by Fiona Kelly and co-authors.
