Don’t be deterred from considering blocks for your rib fracture patients just because you’re not an anaesthetist! We’ve been using SAP blocks @sydneyhems.bsky.social for a few years in our PHARM population #FOAMed #MedSky #LTC2024

Photo of sunrise over the Serpentine lake in Hyde Park - the sun is low in the sky and there are waterfowl visible, including a cormorant standing with wings spread. The sky is clear and the morning is crisp.
Idyllic walk to #LTC2024 this morning through Hyde Park. Improving beauty:death ratio at every opportunity (h/t @airwaycam.bsky.social)

From Prof Ellis keynote at #ltc2024. Everything above the red line can not be learned. They are either there or not. (and I tend to agree!)
Quote of the day from #ltc2024 from Prof Ellis' keynote lecture:
"Procrastination in the name of reducing risk actually
increases risk.
- Once the information is in the 40 to 7O range, go with your gut"
For his second talk at #LTC2024 Prof Demetri Yannopoulos focused on systems issues in the delivery of prehospital ECMO. I’ll highlight some of the key points below.

Agree - nice for senior clinicians to model that it's ok to talk about these issues by sharing their personal experiences.

Prehospital providers are more at risk because we have less control over the workload we experience and we formulate memories that associate unpleasant cases with particular locations we may regularly revisit.

Beautiful and honest talk by
@rod-mackenzie.bsky.social at #LTC2024 on the challenges of wellness for prehospital care providers - as someone who has shared my experiences of postnatal anxiety, I’m glad we are talking about this more openly.

Number 1 reason to use videolarygoscopy i prehospital critical care: fun!
According to Dr Gellerfors at #ltc2024
If you find cooperating with your neighbouring EMS system is challenging, know that someone has it worse. The EMS system in Minesota is a real patchwork of local EMS systems and operators with individual interests and motivations. Dr Yannopoulos at #ltc2024
Hypothermia is an independent predictor of mortality in burns patients - how good are we at measuring temperature in the prehospital environment? Spoiler - not great!

Very important reminder: hypothermia is a big killer in severe burns. Local cooling with running water is effective for outcome, but burns patients loose thermoregulation, so keep warm. Dr Stiles at #ltc2024
Nice reminder for what to focus on in pre-burn centre care (aka prehospital). From Dr Stiles at #ltc2024
Caution interpreting data showing many pts intubated for burns were extubated within 24-48h - this does NOT mean intubation was inappropriate or unnecessary. There are other factors beyond the burn itself informing the decision to intubate in the prehospital environment.
