Development of a Collaborative Education Program for Home Care Nursing Educators’ on-the-Job Training in End-of-Life Home Care: #Protocol for a #RCT #ClinicalTrial Background: Despite the growing demand for end-of-life home care, nursing educators responsible for on-the-job training face substantial barriers, such as time and geographical constraints, which limit their access to professional development. Collaborative online training programs offer a potential solution to these challenges. Objective: This #Study aims to develop and evaluate a collaborative online training program designed to improve the educational ability of home care nursing educators in delivering effective on-the-job training for home-based end-of-life care. Methods: This parallel group, waitlist-controlled trial recruited home care nursing managers or educators from agencies with at least one home death client in the past year. We will provide this program on the internet. The intervention will comprise three online sessions, each lasting 60 minutes and held monthly, involving one lecture and two interactive case studies related to on-the-job training in end-of-life home care. This will be followed by 8 weeks of weekly educational messages via LINE, a mobile messaging #App widely used in Japan. The control group will serve as a waitlist and will receive the same intervention after the control period. The primary outcomes are participants’ attitudes, measured using the End-of-Life Nursing Education Questionnaire. Using a centralized computer-generated randomization system, 125 home care nursing managers and educators will be recruited and randomly assigned to either the intervention or control group at a ratio of 1:1. Stratification will be based on their agency’s annual home death count between November 29, 2023, and November 29, 2024. To minimize performance bias, the intervention content has been standardized before the intervention to ensure fidelity. Results: This #Study was approved by the Ethics Committee of the Graduate School of Medicine, The University of Tokyo (2024359NI-(2)) on November 15, 2024. Participant recruitment began on November 29, 2024, and closed on December 25, 2024, with 120 participants enrolled. As of February 15, 2026, the following number of surveys were completed at each time point: 106 at T0 (the intervention group:the control group; 52:54); 103 at T1 (51:52); 102 at T2 (50:52); 40 at T3 and 43 at T4 in the intervention group; and 47 at T5, 44 at T6, and 41 at T7 in the control group. Quantitative data collection was completed by January 2026, and qualitative interviews will be completed by March 2026. Primary quantitative results and integrated results are expected to be submitted for publication in 2026-2027 and 2027-2028, respectively. Conclusions: This #Protocol can address geographical and time constraints by providing flexible, online educational resources for home care nursing educators. The anticipated outcomes include enhanced on-the-job training ability among nursing educators for home-based end-of-life care. This #Study’s findings could support broader implementation of scalable training strategies in home care settings. Tunnel Anastomosis vs Double-Tract Jejunal Interposition Reconstruction After Proximal Gastrectomy: #Protocol for a Multicenter Prospective #RCT #ClinicalTrial Background: Tunnel anastomosis is a novel anastomotic technique for digestive tract reconstruction following proximal gastrectomy. A previous retrospective #Study by our team demonstrated its favorable antireflux effect; therefore, we hypothesize that tunnel anastomosis is noninferior to double-tract jejunal interposition reconstruction in preventing postoperative reflux esophagitis, and we will conduct this prospective #Study to further validate this assumption. Objective: In this #Study, we will prospectively compare tunnel anastomosis with the currently more prevalent double-tract jejunal interposition reconstruction technique to further validate its safety and efficacy. Methods: This is a multicenter prospective randomized controlled #Study that will enroll 240 patients who will undergo proximal gastrectomy. The #Study will be divided into 2 groups: the tunnel anastomosis group and the double-tract jejunal interposition reconstruction group, with 120 patients in each group. Patients will undergo clinical assessments and complete questionnaires preoperatively, as well as at the 3rd, 6th, and 12th months postoperatively. The primary end point is the incidence of reflux esophagitis within 1 year. The secondary end points include perioperative safety, postoperative quality of life, and postoperative nutritional status. Results: Recruitment of patients commenced in March 2022 and is scheduled to conclude in February 2027. The follow-up for all enrolled patients will be completed by February 2028. Conclusions: To our knowledge, this is the first prospective #Study on this technique, aiming to provide novel insights into the methods of digestive reconstruction following proximal gastrectomy. Impact of Enhanced Family Education on BMI Changes in Children and Adolescents With Overweight or Obesity: #Study #Protocol for a City-Wide Cluster #RCT #ClinicalTrial Background: The prevalence of obesity among children and adolescents has become a critical global public health issue, particularly in China, with significant increases observed over the past few decades. Despite regular surveillance for overweight and obesity in the past decades, there have been no annual reports provided to parents, resulting in a significant underrecognition of this issue. Early intervention and enhanced family-based educational intervention are necessary to address this growing problem. Objective: This #Protocol aims to evaluate the effectiveness of an enhanced family-based educational intervention in reducing overweight and obesity among school-age children and adolescents. Methods: This #Study uses a stratified cluster #RCT #ClinicalTrial design involving 40 schools from 4 cities in Guangdong Province, selected based on economic levels and overweight and obesity prevalence. Schools will be randomly assigned to intervention or control groups. The intervention group receives an enhanced family-based educational intervention on obesity prevention, including health reports and educational materials. The control group continues with regular practices, including routine physical examinations, general health education activities, and school-based physical activity programs. The intervention spans 9 months, followed by a 3-month follow-up. Data on BMI, waist circumference, and waist-to-height ratio are collected at baseline and after 12 months to assess the potential effectiveness of the intervention. Results: This trial was funded in January 2024 and registered in the Chinese Clinical Trial Registry on November 5, 2024. Recruitment was completed in December 2024, with 20 schools enrolled in each of the intervention and control groups. Baseline data collection was completed during the 2024 fall semester, and follow-up data collection continued through December 2025. Data analysis will start after completion of the 12-month assessments. The #Study findings are expected to be published in 2026. Conclusions: This #Study #Protocol addresses the urgent need for effective interventions to combat the increasing prevalence of childhood and adolescent obesity in Guangdong Province. Given the significant underrecognition of the need for consistent reporting to parents, the proposed enhanced family-based educational intervention aims to fill this gap by raising parental awareness and promoting healthier lifestyles among children and adolescents. If successful, this approach could significantly reduce the risk of overweight and obesity in Chinese populations, which account for approximately one-fifth of the world’s population. The findings will provide insights into the efficacy of family-centered interventions and underscore the importance of integrating routine parental reporting into existing surveillance programs. A Culturally Tailored Mobile Health Intervention to Improve Quality of Life in Black Survivors With Prostate #Cancer: #Protocol for a Stratified #RCT #ClinicalTrial Background: Prostate #Cancer (CaP) disproportionately affects Black men in the United States, leading to significant disparities in incidence, survival, and quality of life (QoL). Treatment-related side effects, including urinary dysfunction, pain, fatigue, and psychological distress, contribute to poor long-term outcomes. There is an urgent need for culturally-tailored, technology-based interventions to support symptom self-management and survivorship care. Objective: This #Study aims to develop, refine, and evaluate the Survivorship #App for Ethnically Diverse Black Prostate #Cancer Survivors (SAFE-CaPS), a tailored mobile health intervention designed to enhance QoL, improve symptom self-management, and provide psychosocial support. Methods: This 12-month stratified randomized clinical trial will enroll 248 Black survivors with CaP (including native born, African-born, and Caribbean-born men) into the SAFE-CaPS intervention arm and standard care (SC) control group (1:1 allocation). Eligible participants are self-identified Black men (including native born, African-born, and Caribbean-born) aged 40 to 80 years with a confirmed CaP diagnosis within the past 5 years who own or are willing to use a #Smartphone #mHealth. SC will consist solely of routine oncology or primary care follow-up, including clinic visits, referrals, and supportive services as needed, with no #Digital components. The SAFE-CaPS intervention, developed using the Insight platform, includes daily and weekly ecological momentary assessments of pain, urinary and bowel symptoms, sleep, fatigue, sexual function, mood, physical activity, and diet; adaptive educational modules addressing treatment-related side effects and culturally grounded survivorship concerns; automated symptom alerts prompting nurse follow-up and provider notification; and engagement features such as reminders, goal-setting, and literacy-appropriate content. Intervention content incorporates culturally tailored messaging, patient narratives, and feedback from prior qualitative work with ethnically diverse Black CaP. Stratified randomization will ensure balanced representation of ethnic subgroups. Primary outcomes include overall QoL. Secondary outcomes include specific symptom domains, mental health (depression and anxiety), health care engagement, and #App acceptability. Assessments will occur at baseline, 3, 6, 9, and 12 months, and linear mixed models with intent-to-treat principles will evaluate intervention effects over time. Results: The #Study was funded in 2024, institutional review board approval was received in mid-2024, and the #Study began recruitment on August 24, 2025. Overall, 6 participants have been enrolled, with 2 in the SAFE-CaPS intervention and 4 in SC. Recruitment is expected to conclude in July 2027. Data analysis will occur in late 2027, with article preparation and publication planned for mid-2028 and early 2029. We hypothesize that SAFE CaPS participants will experience greater improvements in QoL, reduced symptom burden, and higher engagement in care compared to SC. Conclusions: This #Study will provide critical evidence on the feasibility, acceptability, and preliminary effectiveness of a culturally tailored mobile health intervention designed to enhance survivorship outcomes among Black survivors with CaP, informing future large-scale trials and real-world implementation. The TBI-AD/ADRD Caregiver Support Intervention (TACSI): #Protocol of a Pilot #RCT #ClinicalTrial Evaluation of a Remote Intervention for Family Caregivers Background: Across the United States, there are millions of informal (ie, unpaid) caregivers helping individuals with Alzheimer Disease/Alzheimer Disease and Related Dementias (AD/ADRD), traumatic brain injury (TBI), or both. TBI is a risk factor for developing, and often co-occurs with, AD/ADRD. In the next decade, more informal caregivers will have to navigate the complexities of the dual diagnosis of TBI and AD/ADRD. Currently, there is a paucity of interventions for caregivers dealing with this dual diagnosis. Our team designed the TBI-AD/ADRD caregiver support intervention (TACSI) as a support program to meet the needs of those providing care to individuals with dual diagnoses of TBI and AD/ADRD. Objective: This pilot #RCT #ClinicalTrial (RCT) evaluates the preliminary efficacy and successful implementation of TACSI, a 6-session semistructured psychosocial and psychoeducational program, over a 6-month period. Methods: This pilot RCT uses an embedded mixed methods randomized controlled evaluation. Caregivers of individuals with a dual diagnosis of AD/ADRD and a history of TBI are randomly assigned to the TACSI (n=41) or a usual care control condition (n=39). Primary outcomes include perceived burden, caregiver relationship satisfaction, and caregiving mastery. Secondary outcomes include caregiver self-efficacy, well-being, and personal resources. Qualitative elements (up to 20 semistructured interviews at the conclusion of the 6-month evaluation and an open-response item at follow-up surveys) allow for better interpretation of quantitative results and understanding of TACSI’s mechanisms of benefit. Results: Recruitment for the #Study was completed in October 2024. Survey collection and intervention were completed in June 2025. Data analysis will follow. Conclusions: Having a dual diagnosis of AD/ADRD and a history of TBI is relatively common. However, there are currently no caregiver support interventions specific to this dual diagnosis. It is important to support these caregivers as they encounter the unique challenges of managing TBI and AD/ADRD. This pilot RCT will help determine if the TACSI program can help to increase caregiving skills and strategies and provide support to improve mood and reduce stress for caregivers of individuals with AD/ADRD and a history of TBI, and also determine if it is feasible to proceed to a full-scale RCT. Comparison of Uncemented and Hybrid Hip Arthroplasty: #Protocol for a Brazilian #RCT #ClinicalTrial Background: Total hip arthroplasty is a highly successful procedure for treating hip arthritis, improving patients’ pain, function, and quality of life (QoL). Scarce publications on total hip arthroplasty performance using Brazilian-manufactured materials report results comparable to those from other countries. Objective: The aim of this #Study is to compare the clinical, radiographic, functional, pain, and QoL outcomes of patients who underwent surgery with a national hybrid prosthesis (with femoral cementation) versus patients who underwent surgery with a national uncemented prosthesis. Methods: This #Study is a single-center, single-surgeon, single-approach, pragmatic, double-blinded, and prospective randomized trial. A total of 120 patients will be enrolled and randomly allocated in a 1:1 ratio to 2 groups: the hybrid group and the uncemented group. The incidence of complications (during and after surgery) and the restoration of normal anatomical parameters on postoperative radiographs will be assessed using radiographic parameters. Participants’ QoL, joint mobility, function, and satisfaction will be evaluated using the 12-Item Short Form Health Survey version 2 questionnaire, hip range of motion, the Harris Hip Score, and a numeric rating scale. Prosthesis survival will be analyzed using the annual revision surgery rate from 1 to 5 years, and up to 10 years. Results: The project did not receive external funding. Data collection began in October 2024 and is ongoing, with completion expected in December 2029. As of April 2025, 72 participants have been enrolled. Preliminary data analysis has been initiated and is ongoing. The first results are expected to be published in the first half of 2026. Conclusions: This #Study is a pragmatic clinical trial that uses blinding to evaluate national implants for both hybrid and uncemented hip arthroplasty. Successful completion of this #Study may provide clinical evidence on the performance of national implants and identify a preferred implant construct (hybrid or uncemented), if any, for hip replacement in Brazil.
