Shared decision-making

Following the diagnosis of a cSDH, efforts should be made to share decisions around treatment options with patients and their advocates. They should be provided with sufficient information to inform decision-making, including an understanding of the risks and benefits of different treatment options.

Indications for surgery

Surgery should be considered in patients with symptomatic cSDH

Consider surgery in patients with minimal or no symptoms of a cSDH, but with radiological evidence of a large volume cSDH with mass effect (e.g. significant midline shift >5 mm).

The thing I love about #realisticmedicine is that when we do what matters most for patients, the evidence is we spend less.
It's all gone back to front in pathway driven care.

WHAT YOU NEED TO KNOW
• Discuss with patients the importance of lifestyle modification for the long term management of acid reflux disorders
• Discuss with patients that a prescription may be stopped even if it has improved symptoms
• Side effects of proton pump inhibitors (PPIs) include gastrointestinal disturbance, higher rate of fractures, and increased susceptibility to Clostridium difficile infection
• Review new prescriptions of PPIs at four weeks, and regularly thereafter, assessing for improvement in symptoms and quality of life

Good to see proton pump inhibitors (drugs like omeprazole and lansoprazole) discussed here in the British Medical Journal.

They aren’t harmless, in a multitude of ways, yet many people end up on them long term.
