This study is the first single-cell atlas to identify CXCL12-high endothelial/fibroblast subtypes in IgG4-chronic rhinosinusitis (CRS). CXCR4 overexpression in T/B cells drives pathological immune hyperactivation, and the CXCL12-CXCR4 axis mediates lymphocyte recruitment in IgG4-CRS. This study proposes the use of CXCR4-targeted therapy for precision intervention of IgG4-CRS. This study investigates the longitudinal effects of #dupilumab on the nasal passage and gastrointestinal microbiota in patients with diffuse type 2 chronic rhinosinusitis with nasal polyps (#CRSwNP). CRSwNP patients show altered nasal microbiota composition compared to healthy individuals. Dupilumab treatment is associated with a shift of nasal microbiota in CRSwNP, with increasing beneficial genera (Lawsonella, Corynebacterium, and Dolosigranulum). Gastrointestinal microbiota remains unchanged during dupilumab treatment.
