Large VA Study Suggests GLP-1 Medications Like Ozempic May Reduce Risk of Multiple Substance Use Disorders and Related Harms A new observational study analyzing electronic health records from over 600,000 U.S. veterans with type 2 diabetes, published in The BMJ, indicates that GLP-1 receptor agonist medications—such as semaglutide (Ozempic, Wegovy) and tirzepatide (Mounjaro, Zepbound)—are associated with significant reductions in addiction-related outcomes. These drugs, originally developed for diabetes and obesity, appear to dampen cravings by acting on brain reward pathways involving dopamine. Among patients with existing substance use disorders, those on GLP-1 drugs experienced 50% fewer substance-related deaths, 39% fewer overdoses, 26% fewer drug-related hospitalizations, and 25% fewer suicide attempts over three years, equating to about 12 fewer serious events per 1,000 people. For those without prior addiction, the drugs were linked to an 18% lower risk of developing alcohol use disorder, 25% lower for opioids, and around 20% lower for cocaine and nicotine dependence, preventing roughly 6-7 new diagnoses per 1,000 users. Animal studies and smaller human trials support these findings, showing reduced alcohol intake and drug self-administration. While not yet approved for addiction treatment, the widespread use of GLP-1 drugs in primary care could offer broad benefits if confirmed by randomized trials. Potential concerns include rebound effects upon discontinuation and impacts on general motivation, but the results highlight a promising repurposing of these medications for a major public health gap in addiction care.
