Factbox-Inside the cockpit: How Air India’s Boeing Dreamliner flight ended in disaster (Reuters) -A cockpit recording of dialogue between the two pilots of the Air India flight that crashed last month indicates the captain cut the flow of fuel to the Boeing (NYSE:BA) 787 jet’s engines, the Wall Street Journal reported on Wednesday. Here is the sequence of events - by the seconds - on June 12, as detailed by Indian investigators in their preliminary investigation report released on Saturday: 05:47 GMT (11:17 a.m. IST) – Air India Dreamliner VT-ANB landed in Ahmedabad from New Delhi as AI423. 07:48:38 GMT – The aircraft was observed departing from Bay 34 at the airport. 07:55:15 GMT – The aircraft requested taxi clearance, which was granted by air traffic control; a minute later the aircraft taxied from the bay to Runway 23 via Taxiway R4, backtracked and lined up for take-off. 08:02:03 GMT - The aircraft was transferred from ground to tower control. 08:07:33 GMT – Take-off clearance issued. 08:07:37 GMT - The aircraft started rolling. 08:08:39 GMT – Aircraft lifted off. "The aircraft air/ground sensors transitioned to air mode, consistent with liftoff," the report said. 08:08:42 GMT – Aircraft reached max airspeed of 180 knots. "Immediately thereafter, the Engine 1 and Engine 2 fuel cutoff switches transitioned from RUN to CUTOFF position one after another with a time gap of 01 sec." "The Engine N1 and N2 began to decrease from their take-off values as the fuel supply to the engines was cut off. "In the cockpit voice recording, one of the pilots is heard asking the other why did he cutoff. "The other pilot responded that he did not do so. "The CCTV footage obtained from the airport showed Ram Air Turbine (RAT) getting deployed during the initial climb immediately after lift-off. "The aircraft started to lose altitude before crossing the airport perimeter wall." 08:08:47 GMT – Both engines’ values "passed below minimum idle speed", and the RAT hydraulic pump began supplying hydraulic power. 08:08:52 GMT – Engine 1 fuel cutoff switch transitioned from CUTOFF to RUN. 08:08:56 GMT – Engine 2 fuel cutoff switch also transitions from CUTOFF to RUN. "When fuel control switches are moved from CUTOFF to RUN while the aircraft is inflight, each engines full authority dual engine control (FADEC) automatically manages a relight and thrust recovery sequence of ignition and fuel introduction." "Engine 1’s core deceleration stopped, reversed and started to progress to recovery. Engine 2 was able to relight but could not arrest core speed deceleration and re-introduced fuel repeatedly to increase core speed acceleration and recovery." 08:09:05 GMT – One of the pilots transmitted “MAYDAY MAYDAY MAYDAY". 08:09:11 GMT – Data recording stopped. 08:14:44 GMT – Crash fire tender left the airport premises for rescue and firefighting. What you need to know about the Air India Boeing Dreamliner crash NEW DELHI (Reuters) -The sudden movement of engine fuel switches has taken centre stage in the investigation of the Air India Boeing (NYSE:BA) Dreamliner crash that killed 260 people in June, the world’s worst aviation disaster in a decade. The crash poses a fresh challenge for Air India, which has long sought to become a "world-class airline", and Boeing, which is working to restore public trust after a series of safety and production crises. THE INVESTIGATION ** A 15-page preliminary report revealed some of what happened in the cockpit and depicted confusion shortly before the jet crashed, after the switches flipped almost simultaneously. It didn’t say how that happened. ** One pilot was heard on the cockpit voice recorder asking the other why he cut off the fuel. "The other pilot responded that he did not do so". ** Around 20 seconds later, one of the pilots gave a "MAYDAY" call. ** The two pilots were Captain Sumeet Sabharwal and First Officer Clive Kunder. They had a total flying experience of 15,638 hours and 3,403 hours, respectively. ** In one internal memo, Air India CEO Campbell Wilson said the report raises additional questions about the incident, urging "everyone to avoid drawing any premature conclusions." ** The final report into the crash could take another year. ** The father of one of the 260 people killed in the Air India crash called for punishment of those responsible after the release of a preliminary report about the disaster. THE FALLOUT ** The focus is on engine fuel switches that regulate fuel flow into a plane’s engines. They are used by pilots to start or shut down engines on the ground or to manually shut down or restart engines if an engine failure occurs during a flight. ** The U.S. Federal Aviation Administration and Boeing have privately issued notifications that the fuel switch locks on Boeing planes are safe. ** But that has not stopped India and South Korea from ordering their airlines to check fuel switches on several Boeing models. Singapore Airlines (OTC:SINGY) also said it was making precautionary checks. Air India hasn’t found any problems yet. ** Flying anxiety has peaked in India. Some travellers are becoming more choosy in selecting their airline and aircraft. CCTV FOOTAGE, TRAGIC TALES ** Air India and its low-cost unit have come under intense scrutiny since the crash from the Indian aviation regulator due to maintenance lapses. * There were 169 Indians, 53 Britons, 7 Portuguese, and one Canadian among the passengers on board. Twelve crew members were also on the plane. * The only survivor was Viswashkumar Ramesh, 40, who was in seat 11A, near the emergency exit. * The most chilling footage of the crash was a 59-second clip that showed the takeoff and crash into a huge fireball. * The plane hit a college hostel building. Wheels and other parts of the aircraft were embedded in the walls. Nineteen people on the ground were killed. * Dozens of anxious family members waited for days to collect bodies of loved ones as doctors work to gather dental samples and do DNA profiling to identify bodies badly charred. What you need to know about the Air India Boeing Dreamliner crash (Reuters) -The crash of an Air India Boeing (NYSE:BA) 787 Dreamliner seconds after takeoff from Ahmedabad city, killing all but one of the 242 people on board, is the world’s worst aviation disaster in a decade. At least 30 people were also killed on the ground as the airliner crashed into a medical college hostel near the airport. The crash poses a fresh challenge for Air India, which has long sought to become a "world-class airline", and Boeing, which is working to restore public trust after a series of safety and production crises. CRASH, ANXIOUS FAMILIES AND RESCUE * A 59-second CCTV video clip, one of the strongest pieces of visual evidence, showed the takeoff and the crash. The descent of the plane starts roughly 17 seconds after takeoff. There is no fire visible around the engine or elsewhere when the plane appears to start going down. * There were 169 Indians, 53 Britons, 7 Portuguese, and one Canadian among the passengers on board. Twelve crew members were also on the plane. * The only survivor was Viswashkumar Ramesh, 40, who was in seat 11A, near the emergency exit. * Dozens of anxious family members are waiting to collect bodies of loved ones as doctors work to gather dental samples and do DNA profiling to identify bodies badly charred. EARLY INVESTIGATION * Air India and the Indian government are looking at issues linked to engine thrust, flaps, and why the landing gear remained open. The black boxes, crucial to the crash probe, have been recovered. * India’s aviation safety watchdog has asked Air India for the training records of the pilots and dispatchers, while asking flying schools to conduct training compliance checks. * Officials from the U.S. National Transportation Safety Board and U.S. Federal Aviation Administration officials, as well as Boeing and GE, have surveyed the crash site. * An inspection of Air India’s 787 fleet did not reveal any major issues, but the Indian aviation watchdog raised concerns about recent maintenance-related issues reported by the airline and advised the carrier to "strictly adhere to regulations". TRAGIC TALES * The plane hit a college hostel building. When Reuters visited, some steel tumblers and plates containing food lay on the few tables that were left intact. Wheels and other parts of the aircraft were embedded in the walls. * Ravi Thakor, a cook at the college hostel, and his wife have been praying for a ’second miracle’ - just like the survival of the sole passenger - as he searches for his two-year-old daughter, whom he had rocked to sleep before stepping out 30 minutes before the crash to deliver lunch boxes. * Lawrence Christian, a 30-year-old working in Britain, had flown to India to bury his father, but just two weeks later, his family will have to bury him. His grandmother grieved the loss of "light of our home." INSIDE AIR INDIA * Boeing Commercial Airplanes’ head Stephanie Pope visited India and met Air India Chair N. Chandrasekaran at the airline’s headquarters. * In another incident, an Air India Boeing 787-8 Dreamliner plane bound for New Delhi returned to its origin of Hong Kong after takeoff on June 16 following a technical issue. BA: is this perennial leader facing new challenges? With valuations skyrocketing in 2024, many investors are uneasy putting more money into stocks. Sure, there are always opportunities in the stock market – but finding them feels more difficult now than a year ago. Unsure where to invest next? One of the best ways to discover new high-potential opportunities is to look at the top performing portfolios this year.
