Ponified version of Sally from 'Play With Me' creepypasta. She is depicted as a peach to tan-colored earth pony filly with glowing green eyes wearing a pink nightgown and carrying a brown teddy bear with a missing right eye called 'Mr. D' or 'Mr. Death'. Her mane/tail are light brown and floating ethereally as she is, as she is a ghost. A bit of pink brain matter is exposed on the top of her head. Her hooves and nightgown as caked in mud and there are bruises and cuts on her body. She bleeds from a head wound and from her eyes as if they were tears.
