His arm is around Sam's side, probably gripping his back. Castiel is wearing one of Sam's t-shirts, that is kinda large on him. The second sketch is a copy and paste, except that now Sam's head downs to kiss Castiel's forehead and that Cas leans up to the affection, still crying a little. Mr. I-don't-require-sustenance always hogs the blanket. It's worse when Jack is there. Sam is left without a blanket even if they get another one. It's why Sam has chosen to be in the middle if Jack is there. (They still managed to hog the blankets.)
