Highlights from late-breaking @TheLancet at #ESCMIDGlobal2026 📊
🔹 WGS + AI for RR-TB → better outcomes
🔹 UTI → nitrofurantoin best; fosfomycin worst
🔹 H5 immunity → partial cross-protection
🔹 M. pneumoniae CAP → steroids speed recovery
🚀 Precision + smarter antimicrobial use are shaping ID
Posts by The Lancet Microbe
Key take-home at #ESCMIDGlobal2026:
🧬 Partial cross-reactive immunity to H5 exists
💉 Likely from prior seasonal influenza exposure
⚠️ May not prevent infection—but could blunt severity
👉 Population not fully naïve to H5N1 👀
New data at #ESCMIDGlobal2026:
🧬 Functional N1 antibodies detected against both seasonal & avian influenza
🔗 Strong correlation between responses
💉 Likely driven by prior H1N1 exposure (infection or vaccination)
👉 Population may be less immunologically naïve than assumed
H5 spillover is increasing—but the key question is immunity.
🦠 Frequent spillover: poultry → mammals → humans (no sustained transmission)
⚠️ Pandemic risk hinges on spread in an immunologically naïve population
👉 Are we more vulnerable than we think? #ESCMIDGlobal2026
H5 avian influenza at #ESCMIDGlobal2026:
🦠 Expanding host range → birds, mammals (incl. cattle), occasional human cases
🌍 Now across 6 continents with rising spillover
⚠️ Pandemic risk = spread in an immunologically naïve population
No sustained human transmission—yet 👀
Next Up! at #ESCMIDGlobal2026:
🦠 Population immunity to highly pathogenic avian influenza (H5) in healthcare workers
Early insights into immune responses—and what this means for exposure risk and pandemic preparedness 👀

What drives better outcomes?
Receiving ≥3 susceptible drugs was key to treatment success & very short regimens (≤9 months) were linked to worse outcomes.
Optimising regimen strength & tracking relapse-free survival should be central to MDR-TB care.
Multidrug-resistant #TB (MDR-TB) outcomes may be better than we think.
In a national cohort from Latvia, only 4.8% met WHO-defined cure—but 76.9% achieved long-term relapse-free survival.
