4) Recommendations Non-maleficence Uilize cerebral blood monitoring to avoid the possibility of experiential patient harm, as recommended by the European Society for Organ Transplantation. Engage solely in controlled NRP procedures (in which life sustaining therapy is withdrawn) to avoid harms to patients, families, and other stakeholders. Beneficence Engage in NRP to increase the donor pool, improve the viability of anatomical gifts, and the number of anatomical gifts that can be procured from a donor, in contrast with alternative procurement methods (e-g, ex-vivo DPP). Only permit NRP incaseswhere patients can no longer be the target of medical beneficence (e.g. when care is futile, such as in controlled WLST). Justice • NRP should be coupled with a just distribution schema, such as a lottery, a weighted lotery, or UNOS "continuous distribution" (which began in 2023) to serve distributive justice considerations. • NRP should not subjcctprospectivedonors to unjust processes that affect their nedical care, and NRP should not lead hospitals to problematically seek out or discriminate against members of disadvantaged groups (eg. those suffering from the overdose epidemic). Autonomy • Abide by patient choices (or justified proxy decisionmakerchoices) regarding anatomical giving. Explain TA-NRP thoroughly such that authorization and consent are obtained. Only use organ reperfusion in keeping with a patient's stated goals; if a patient has a DNR order within their advance directive, NRP it mayt be impernissible. The definition of resuscitation is contested, but NRP has been claimed to cause resuscitation, so it is best to avoid it in such cases. Conclusion: These principlist guidelines providea path forward for morally permissible NRP.

Dr. Jack Harris defended controlled Thoracoabdominal Normothermic Regional Perfusion (NRP) if treatment can no longer help patients, patients' goals align, and cerebral blood is monitored.
