Could Therapists Be Harming Clients With OCD Without Realizing It?
Unintentional harm in OCD treatment often comes from good intentions and mismatched tools—not unethical intent.

Supervision is where you grow. It's the critical space to process difficult cases, check your ethical blind spots, and manage your own clinical baggage. If you're not challenged in supervision, you're not utilizing it fully. 🧠

Does it matter what a therapist wears?

Julie Sale & Marcelline Menyié team up to explore this ethical question. It’s about transference vs. authenticity, and why what you wear is never neutral.

What you wear is an extension of identity.

Are we running ahead of the client? 🤔

The pressure to "fix" or find solutions can make us sprint past where the client actually is. Ethical practice means respecting their pace. Slowness is a clinical tool.

What's the hardest word to say in the room? 🤐

Ethical practice demands that we name and address systemic issues like "power", and "privilege". We train therapists to develop the courage to speak the unspeakable.

In Psychology we pay such close attention to ethical practices & rigour in research- why is this different for @ouhospitals.bsky.social @oxfordlawfac.bsky.social @oxprimarycare.bsky.social?

July ‘24 I complained to @ox.ac.uk re the use of patient data collected via MNVP FB survey w/o ethical approval or patient consent by academics, #OxfordUniversityHospitals staff & the MNVP (funded by NHS BOB ICB)- jme.bmj.com/content/51/5/j…

Oxford University said no issues…🤷‍♀️

And yet while women are ignored & not listened to by maternity staff at OUH, their data is used for research projects, & they are suddenly of interest when the harm they experienced can be ‘showcased’ in medical journals.

“Don’t Tell Them Anything”: Should Surrogate Decision-Makers Be Allowed to Withhold Information from Other Family Members or Prevent Them from Visiting with a Patient? - HEC Forum While patients have the right to control who has access to their health information and designate visitors, it is not always clear whether—when a patient lacks capacity—their surrogate also exercises such rights. States and federal laws are often vague about the limits of surrogate authority. Even where legal or institutional guidance on this issue is clear, requests by surrogates to withhold information or restrict visitation with a patient can be a source of ethical uncertainty and distress on the part of the clinical team. This paper explores the ethical issues raised by such requests. To date, there has been little exploration of this issue in the clinical ethics literature. First, we summarize the scant existing ethical and legal guidance on this issue. Second, we present two potential approaches to navigating requests from surrogates to withhold information or restrict visitation. Third, we discuss the merits and limitations of both approaches, and introduce some additional considerations that further complicate the picture. We argue for a flexible restrictive approach to information-sharing, and a constrained permissive approach to visitation. Finally, we propose several considerations that clinicians and clinical ethicists might think through in these situations to help guide their practice.

🚨 our new article is out now in HEC Forum! in it, we explore how to navigate requests by surrogate decision-makers to withhold information from other family members or prevent them from visiting with a patient 🚨 #clinicalethics #medicaldecisionmaking

I’m genuinely curious to hear from those working in this space—ethicists, geneticists, clinicians. Am I missing key safeguards or good practice models? Open to being corrected/challenged. I’m here to learn as much as to provoke.

Genuine question: are all parents being offered genetic counselling before enrolling in these studies? Are there enough trained counsellors to do this well? If not, who is helping them interpret what these tests might mean—for now and for the future?
