Potential Application Value and Needs of Using Generative AI Videos for Health Management for Older Adults: Qualitative Study Background: Population aging has emerged as a global concern, older adults’ ability to access health knowledge and manage their well-being impacts their health outcomes. In the artificial intelligence era, generative artificial intelligence (GenAI) videos hold promise for enhancing geriatric health management. However, their potential and the needs of older adults in using GenAI videos for health-related purposes deserve a more in-depth investigation. Objective: This study aims to explore the application potential and multifaceted needs of older adults in using GenAI videos for health information acquisition and management, while providing actionable recommendations for future aging-friendly GenAI video tools. Methods: A qualitative approach was adopted. Twenty older adults (aged ≥60 y) with basic digital literacy were recruited from communities for participation in iterative GenAI video workshops. Semistructured interviews were conducted. Thematic analysis, following Braun and Clarke’s 6-phase reflexive framework, was used to identify key themes from interview transcripts. Results: Our results have identified a 3-layer hierarchical structure of needs when older adults interact with GenAI videos. The first layer is technical accessibility, which suggests the direct barriers preventing them from operating GenAI tools independently. The second layer is age-friendly design, which depicts the needs of age-friendly design adaptations, which can help older adults to better use GenAI features. The final layer is integration with professional resources, highlighting the need for integrating professional medical information resources with GenAI tools, so that the GenAI videos can be more appropriately used for health management purposes. By triangulating the core needs of older adults, the capabilities of GenAI, and their remaining gaps, we have found that the existing gaps limit the extent to which the health needs of older adults are met, and also inversely restrain the acceptance and use of GenAI videos by older adults. Conclusions: This study explored the potential and gaps of using GenAI videos in older adults’ health management. GenAI videos are prominent for self-management of health, but our triangular model reveals coexisting opportunities and challenges. While the core needs drive the development directions of GenAI video technologies, our study suggests that GenAI health videos can benefit from the improvements in technical capabilities, service innovation, localization, and integration of health resources.

Evaluating the Use of Generative AI Videos for Health Self-Management of Older Adults: Mixed Methods Study Background: Aging is a pressing global issue, and older adults need to build up their knowledge to manage their health. Insufficient self-efficacy and low acceptance of technology hinder their ability to use emerging technologies for self-management. Objective: This study explored the potential of generative artificial intelligence (GenAI) videos in the health management of older adults. Methods: We developed a video-based GenAI prototype, AIHealthV. This study used a mixed methods approach, enrolling 20 older adult participants (aged 60 to 80 years) in 3 rounds of iterative workshops. Data collection included pre- and postquestionnaires, in-depth interviews, prompt text records, and the generated video content. Qualitative data were analyzed using the 6-stage reflexivity thematic analysis method by Braun and Clarke, and quantitative data were analyzed using the Wilcoxon signed-rank test. Results: The findings revealed that GenAI videos can enhance older adults’ self-efficacy and technology acceptance, reduce cognitive load, and simultaneously meet their health management needs. The multimodal content generated by GenAI makes health information more comprehensible and thus improves the accessibility of health knowledge. Following the workshop, the interaction between older adults and AIHealthV exhibits a trend of exploration, adaptation, and verification. Conclusions: Despite ethical, privacy, and usability concerns, AIHealthV has been proven to be beneficial in improving the health management capabilities of older adults. This paper also provides practical insights for developing artificial intelligence health tools tailored to older adults as GenAI tools continue to prevail.

Impact of a Consumer e-Learning Course on Beliefs, Treatment Choices, and Outcomes Among People With Hip and Knee Osteoarthritis: Qualitative Interview Study Background: First-line management for hip and knee osteoarthritis includes lifestyle treatments, such as exercise and weight loss (if appropriate), whereas joint replacement surgery is recommended only for severe symptoms after these options have been exhausted. However, many people with osteoarthritis hold misconceptions about the condition, leading to lower acceptance of nonsurgical treatments, such as exercise, and the mistaken belief that surgery is their only option. Novel patient education approaches that address these misconceptions are recommended to improve uptake of lifestyle treatments, reduce unnecessary surgery, and improve outcomes for people with osteoarthritis. We developed a 4-week self-directed consumer e-learning course on osteoarthritis management. In a randomized controlled trial, using the course led to immediate and sustained improvements in osteoarthritis knowledge. However, participants’ perspectives on the course and an understanding of how it impacted osteoarthritis beliefs, treatment choices, and outcomes were unknown. Objective: This study aims to explore how an e-learning course for people with hip and knee osteoarthritis may have impacted their osteoarthritis beliefs, treatment choices, and outcomes. Methods: In this qualitative study, we conducted semistructured individual interviews (N=20) with randomized controlled trial participants with hip or knee osteoarthritis who accessed a 4-week consumer e-learning course on osteoarthritis and its management. Interviews were audio recorded, transcribed verbatim, and thematically analyzed following a framework approach, which was guided by the common sense model of self-regulation. Results: Four themes were developed from the interviews: (1) participants reshaped their beliefs and attitudes toward osteoarthritis and its management, (2) participants adopted a proactive approach to management, (3) participants developed a more positive mindset, and (4) the course supported learning and shifts in beliefs. Conclusions: The e-learning course resulted in shifts in participants’ beliefs and attitudes toward osteoarthritis and its management, increasing their confidence in living with osteoarthritis and resulting in a more optimistic outlook on the future. The e-learning course is freely available and could be a useful resource for people with osteoarthritis to enhance their understanding of the condition and its management.

Frequency of Electronic Personal Health Record Use in US Older Adults: Cross-Sectional Study of a National Survey Background: Electronic Personal Health Records (ePHRs) have significant potential to improve health management for older adults by enhancing access to medical information and communication with healthcare providers. However, usage remains low among individuals aged 65 and older. Existing research points to barriers such as low self-efficacy, digital literacy, and usability challenges, but the specific factors influencing ePHR use among the older adults are not fully understood. Objective: This study integrates the Aging and Technology framework and the Patient Technology Adoption Model to investigate key predictors of ePHR use, including age, education, issue involvement, performance expectancy, effort expectancy, and self-efficacy, while controlling for demographic factors such as gender, race, and income. Methods: This study uses data collected from the Health Information National Trends Survey (HINTS 5 Cycle 3) which includes 532 respondents representing 13,136,180 U.S. adults aged 65 and older when survey weights are utilized. The study uses structural equation modeling to analyze the frequency of ePHR access in the last 12 months. Results: Results show that older adults with higher self-efficacy used ePHRs more frequently, and factors such as issue involvement, performance expectancy, and effort expectancy positively influenced ePHR use. Interestingly, self-efficacy also partially mediates the impact of age on the frequency of ePHR use. Conclusions: These findings suggest that improving self-efficacy, focusing on usability, and enhancing perceived benefits are critical for increasing ePHR usage among the older adults. The study underscores the need for targeted interventions to support older users, to simplify interfaces, and to provide educational resources, ultimately improving health outcomes and quality of life for older adults.
