Here's a very suave doctor using the Czermak's #laryngoscope (c1850) with reflector and laryngeal mirror. Using sun or lamp light the reflector (held between the teeth) shines light onto the mirror which could be positioned to illuminate the larynx or nasopharynx/nose.
