Digital Smoking Cessation Preferences of Predominately Low-Income and Latino Residents of the San Joaquin Valley in California: Qualitative Study Background: Although rates of tobacco use in California have declined overall, adults in the San Joaquin Valley (SJV), particularly Hispanic/Latinos (“Latinos”), have disproportionately high rates of tobacco use, tobacco-related illness, and mortality. Residents of the SJV also have limited access to cessation support services, and need accessible, non-clinical alternatives. Given high smartphone use rates among Latinos and residents of rural communities, digital health tools may present an accessible approach to expand cessation support. Objective: This study explored tobacco use behaviors, cessation experiences, and views about digital cessation tools for tobacco cessation among SJV residents. The secondary objective was to assess the appeal, #usability, and necessary adaptations of two existing digital smoking cessation tools—a smoking cessation app and a social media-based intervention. Methods: Through a SJV-based academic-community partnership, we recruited 29 predominantly Latino adults who reported current smoking. We conducted four focus groups (two English, two Spanish) to explore tobacco use and cessation experiences, and preferences for smoking cessation tools. Nine participants subsequently completed in-depth interviews where they viewed videos describing two digital smoking cessation tools — a cessation app and a social media intervention — to assess their appeal and #usability. Results: Most participants were motivated to quit despite experiencing barriers, emphasizing the need for culturally tailored digital cessation tools to enhance engagement. They preferred interventions that integrated culturally relevant content reflecting lived experiences, featured language-concordant communications, and provided social supports, such as chat rooms for peer connection. While participants appreciated the app’s private interface and comprehensive curriculum, the social-media based program was favored for its engaging design, despite privacy concerns. Preferences for specific interventions varied by age and digital literacy. Material rewards increased appeal to use both digital health tools to quit smoking. Conclusions: This sample of predominantly Latino adults from the SJV expressed favorable interest in digital cessation support, yet existing tools require adaptation to improve cultural relevance, accessibility, and #usability. Participants emphasized language-concordant services, representation from people with lived experience, and community-building features. While digital interventions were well received, privacy concerns and digital literacy barriers must be addressed to enhance engagement.

Formative Evaluation of an HIV Prevention App tailored for Latino Men Who Have Sex With Men: Acceptability and #usability Study Background: Human Immunodeficiency Virus (HIV) incidence is increasing among Latino gay, bisexual and other men who have sex with men (LMSM) in the Atlanta metropolitan area. Mobile phone applications represent an innovative tool to promote pre-exposure prophylaxis (PrEP), HIV testing, and condom use. Objective: To assess the acceptability and #usability of Saludfindr, an Android-based HIV prevention app tailored to the needs of LMSM in the Atlanta area. Methods: We recruited adult LMSM to interact with the app for 4 months. Saludfindr included initial and periodic health assessments, provision of suggestions regarding PrEP, HIV testing, and condom use, in-app product ordering, customized motivational messages, a customized sexual health clinic list, and a “Contact Us” button. To assess acceptability, we measured usage of each app feature, PrEP and HIV testing uptake, and participant ratings of the app’s usefulness. We assessed #usability with the System #usability Score (SUS). Results: We enrolled 31 participants; median age was 27 years [interquartile range (IQR) 24.5-32], 97% were cisgender men, 81% (25/31) identified as MSM, and 61% (19/31) used the app in Spanish. All participants completed the initial health screening, with 84% (26/31) and 77% (24/31) completing the 2- and 4-month health screenings, respectively. Of all participants, 52% (16/31) and 23% (7/31) ordered condoms and home HIV tests through the app at least once, respectively. During the study period, 71% (22/31) of participants got tested for HIV, of whom 68% (15/22) accessed it through clinic-based HIV testing. Of the participants not on PrEP at baseline, 41% (7/17) initiated PrEP during the study, and all of them did so at one of the clinics listed in the app. Saludfindr reached a SUS score of 74.5/100 (excellent). Conclusions: Saludfindr was highly acceptable and usable among LMSM participants in the Atlanta area. In-app assistance to access PrEP and clinic-based HIV testing referrals were well received. Further efforts are needed to increase users’ self-efficacy with home HIV self-testing.

Culturally Adapted STAR-Caregivers Virtual Training and Follow-Up for Latino Caregivers of People Living With Dementia: Single-Arm Pre-Post Mixed Methods Study Background: Latino caregivers are at an increased risk of negative health outcomes due to the responsibilities of caring for someone with dementia. Although interventions exist to address caregiver burden, they often do not meet the cultural needs of Latino caregivers. Objective: This study aimed to pilot test the cultural adaptation of the STAR-Caregivers Virtual Training and Follow-Up (STAR-VTF) intervention. The intervention is an evidence-based training program designed to teach family caregivers strategies to manage behavioral and psychological symptoms of dementia (BPSD). Our research team has conducted past studies to identify and perform culturally relevant adaptations to the online learning modules of STAR-VTF, and this study aimed to pilot these culturally adapted learning modules with a sample of Latino caregivers. Methods: Data on feasibility, usability, and acceptability were collected from a pilot test in which Latino caregivers (N=16) used the online learning modules of the STAR-VTF intervention over a 7-week period. Participants completed usability surveys following the completion of each learning module, and acceptability was assessed through semi-structured interviews (N=14) post-intervention. Preliminary outcome measures were also collected, and a descriptive analysis was conducted. The primary outcomes were the Revised Memory and Behavior Problem Checklist (RMBPC) and the Preparedness for Caregiving Scale. Results: The pilot study results suggest that it is feasible to deliver the culturally adapted STAR-VTF intervention to Latino caregivers, with 93.75% (15/16) of participants maintaining enrollment through intervention completion. The intervention's usability was found to be “good” based on an average System Usability Score of 76.7 out of 100 across all learning modules. Caregivers were generally satisfied with the online learning modules. In addition, preliminary outcome results demonstrated a trend of decreased BPSD pre- versus post-intervention (RMBPC sub-scale score: 28.24 to 21.34). Findings also demonstrated decreased caregiver reaction to BPSD pre- versus post-intervention (RMPBC sub-scale score: 40.40 to 37.21) and increased caregiver preparedness based on pre- and post-intervention (Preparedness Caregiving Scale score: 1.98 to 2.43). Conclusions: The pilot study demonstrated that the culturally adapted STAR-VTF intervention is feasible and perceived as easy to use based by a small sample of Latino caregivers. We aim to refine the cultural adaptations of the STAR-VTF intervention further based on feedback from study participants. Future studies are necessary to test the efficacy of the intervention and support the broad dissemination of the culturally adapted intervention.

#feasibility and Cultural Adaptation of a Community-Engaged Physical Activity Intervention for Hispanic Older Adults: Pilot Study Background: Despite facing significant health disparities and higher risk of dementia, culturally adapted interventions to improve brain health in the Latino population are limited. Objective: To culturally adapt a physical activity (PA) intervention for the older and middle-aged Latino community based on participant and community feedback gathered during a short pre/post pilot study. Methods: The cultural adaptation process involved translation of materials, community advisory board (CAB) review, and a pre-post pilot followed by focus groups and thematic analysis to inform the larger trial. Pilot intervention tools included using a Fitbit tracker and two phone calls to set individualized goals with a health coach. #feasibility and acceptability data, both qualitative and quantitative, were collected. Results: Ten Latino volunteers (mean age = 67.2, 90% female, 90% Spanish-speaking) completed the pre-post study and nine attended the focus groups. Completion rate was 100%, recruitment rate was 50%, and the threshold for acceptability was met, with all participants rating agree or strongly agree on the study satisfaction survey. Participants favored group activities and more interaction with the study team. Identified barriers included time constraints, health issues, and weather, while awareness of PA's cognitive benefits, accountability, self-efficacy, enjoyable activities, social support, and Fitbit use were seen as facilitators. Conclusions: Feedback from participants and the CAB was key in culturally adapting the intervention. This pilot work will inform a larger randomized controlled trial to reduce dementia risk in Latino older adults.
