AI-Enhanced Automatic Life Story Structuring for Reminiscence Therapy in Older Adults: Technical Feasibility Study Background: Storytelling interventions have demonstrated substantial potential in improving emotional well-being, cognitive function, and quality of life for older adults. However, its effectiveness is often limited by the challenges of processing disorganized and redundant life stories, which impose substantial cognitive demands on caregivers. Although storytelling interventions are a well-established therapeutic approach, current practices depend heavily on manual narrative organization, restricting both the scalability and consistency of treatment delivery. Prior research has primarily focused on validating the clinical outcomes of storytelling interventions, with insufficient attention given to technological solutions that could enhance narrative processing while preserving therapeutic integrity. Digital approaches to life story structuring remain underexplored, despite their potential to amplify storytelling benefits by reducing cognitive load and improving recall accuracy. Objective: This study aims to design an event timeline generation algorithm to optimize the prior work of the Story Mosaic system. The optimized system enables (1) the automatic extraction of event elements from life narratives, (2) the automatic organization of fragmented life stories into structured timelines, and (3) the preservation of clinically relevant contextual details during compression. The goal is to reduce manual intervention costs while increasing treatment efficacy through artificial intelligence–driven narrative structuring. Methods: We have designed a novel method, CARE event timeline (CARE-ET), which combines a temporal attention mechanism with graph-based event relationship modeling. Furthermore, we used the CARE-ET algorithm to optimize existing story collage systems. The system uses multifeature extraction technology to capture event clues from oral histories, prioritizes the 6 elements of events through a hierarchical attention mechanism, and uses adaptive compression algorithms to reduce redundancy while maintaining narrative continuity. To verify the effectiveness of the CARE-ET method, this paper adopts a multidimensional evaluation framework, which encompasses event summary assessment, timeline quality evaluation, and usability testing of the optimized system. Results: The proposed CARE-ET algorithm outperforms the baseline in both narrative flow and temporal accuracy. The Story Mosaic system, optimized by the CARE-ET algorithm, underwent usability evaluation by 10 caregivers recruited for this study. Based on standardized assessment metrics, the system received an A rating for usability. The comprehensive experimental results demonstrate that the CARE-ET method can effectively structure fragmented narratives from older adults, enhancing the usability of the Story Mosaic system. Conclusions: The proposed method enables the structured extraction of representative event summaries, transforming disorganized life stories into an event timeline for caregiver-supported older adult well-being interventions. Future research should investigate longitudinal effects on cognitive preservation and explore integration with existing dementia care protocols. This work establishes a critical foundation for intelligent assistive technologies in geriatric mental health interventions.

Public-Facing Communication of Health and Social Services for Older Adults and Their Family or Friend Caregivers: Environmental Scan of 58 Integrated Care Teams’ Websites in Ontario, Canada Background: Family or friend caregivers of older adults are critical in helping older adults navigate fragmented health and social systems, but they face significant challenges in doing so. Their needs for support, information, and resources are often unmet or remain largely invisible to health and social systems and public policy. In Ontario, Canada, Ontario Health Teams (OHTs) were established to integrate and streamline health care services. However, emerging evidence suggests that despite the requirement to integrate patient and caregiver advisors in these activities, caregivers still face substantial navigation barriers. Objective: This study aimed to systematically evaluate the amount, nature, and accessibility of information provided on each of the 58 OHT websites. Specifically, we focused on information on services and supports for older adults and their caregivers. Methods: Between November 2024 and May 2025, we conducted an environmental scan of all 58 OHT websites. Using a 5-point Likert scale, 2 team members independently rated how easy or difficult it was to identify services and supports for older adults and their caregivers. They also documented each service and support listed on each website and provided additional details on the experience of navigating the website in an open-text comment. The ratings were discussed in team meetings, and discrepancies were resolved through team consensus. Data analysis included thematic analysis of the services identified and of open-text responses (positive and negative experiences of navigating the websites, rationales for the ratings), as well as descriptive statistics of the ease of access ratings and of the types of services listed on OHT websites. Results: Almost 60% of the websites were rated as difficult or very difficult to navigate, and 33% provided insufficient information on services and supports. However, information quality and accessibility varied significantly between websites. While some featured clear, well-organized resources, others were poorly designed, lacked a well-functioning search function, or provided vague or incomplete descriptions of services and supports. Design features that improved the accessibility and usefulness of websites included user-friendly, simple navigation and direct links to relevant services. In contrast, poorly designed websites often require multiple steps to access essential information, risking exacerbating caregiver burden. Conclusions: Our findings highlight significant barriers for caregivers to access and navigate health and social service information, despite the intended goals of OHTs to improve system navigation. Health care system reforms focusing on integrated care need to include older adults and their caregivers as priority populations. Older adults and caregivers need to be engaged systematically and comprehensively, including in the development, design, and evaluation of health care system websites. Further, standards of public reporting need to be developed, and integrated care networks need to be required to follow these standards. This will help to improve transparency and accountability.

AI-Assisted Dynamic Postural Control Screening to Improve Functional Mobility in Older Adult Populations: Quasi-Experimental Study Background: Falls are a leading cause of injury, disability, and mortality among older adults, imposing significant burdens on individuals and healthcare systems worldwide. Effective fall risk assessment and intervention strategies are critical for improving mobility and preventing adverse health outcomes in aging populations. Traditional assessment methods often require specialized equipment or trained personnel, limiting accessibility and scalability. Objective: This study investigates the effectiveness of an artificial intelligence (AI)-assisted dynamic postural control screening system in predicting and mitigating fall risks among older adults. By integrating AI-driven image analysis with standardized mobility assessments, the study aims to enhance early detection and personalized intervention strategies for fall prevention. Methods: A cohort of elderly participants underwent AI-assisted postural control screening, which incorporated convolutional neural networks (CNNs) with the Short Physical Performance Battery (SPPB) to evaluate balance, gait, and lower limb strength. Participants were divided into an experimental group receiving AI-driven personalized exercise interventions and a control group following standard care. Mobility outcomes, including SPPB scores, gait speed, and sit-to-stand performance, were measured pre- and post-intervention. Results: Our findings suggest that AI-assisted postural control screening effectively enhances mobility in elderly individuals. The experimental group exhibited a 7% increase in SPPB scores, alongside notable improvements in gait speed (+13%) and sit-to-stand performance (-13%). These results indicate that real-time AI feedback may play a critical role in optimizing exercise interventions, as personalized recommendations can be dynamically adjusted based on individual progress. Notably, participants with higher adherence to AI-driven programs showed greater functional gains, underscoring the importance of continued engagement in technology-assisted fall prevention strategies. Conclusions: AI-driven dynamic postural control screening presents a scalable, cost-effective solution for fall risk assessment and prevention in older adults. While the technology shows promising outcomes, challenges such as accessibility, validation across diverse populations, and implementation in low-resource settings remain. Future research should explore long-term effects, broader demographic inclusion, and strategies to overcome technological barriers to enhance widespread adoption in geriatric care.

The Caregiver Support Model for Informal Caregivers of Frail Older Adults: Randomized Controlled Trial Background: Caregivers of frail older adults face substantial challenges, often managing their own health while providing care. To address these issues, we developed the Caregiver Support Model (CSM), a structured approach that uses systematic assessment, personalized intervention planning, and sustained support to address informal family caregivers’ diverse and evolving needs and leverage their resources. Objective: This study aims to evaluate the effectiveness of CSM. Methods: A blinded cluster randomized controlled trial (RCT) was conducted across eight centers providing services for older adults in Hong Kong. The CSM is a social worker-guided intervention that integrates a structured assessment of caregiver needs and resources, personalized service planning, and ongoing monitoring over six months. Meanwhile, the control group continued with their usual procedures without a standardized caregiver assessment. Data were collected at baseline, three months, and six months. Results: We recruited 565 informal family caregivers (281 CSM intervention, 284 standard care control). Both groups improved over time; compared with the control group, the CSM produced greater reductions in caregiver needs, particularly in role conflict, and greater gains in resources, such as health awareness. Improvements were more pronounced at six months compared to three months, indicating a lasting effect and consolidation of gains. The intervention was particularly effective for caregivers in other relationships (not spouse or child) and those with higher education than spousal caregivers. Conclusions: These findings highlight the importance of long-term, tailored interventions that adapt to the evolving needs of caregivers through systematic assessment. The CSM offers a promising approach to enhancing the well-being of caregivers and managing the complex demands of caregiving, particularly in an aging population.
