1) Start with a dark color. Can be black or almost black. I chose a very very dark blue.
2) Airbrush and pick other colors, and use darker version of that color but enough to stand out.

The science of why it looks that way is because skin somewhat translucent, and isn't red by itself, the underlying blood/muscles are red, and when light shines on it, the red shows through on the shadow, but not on the light side.

Doable in #ibispaint or really, any prog with clipping layers & net screentone
Layers > screen tone, resize it a bit if you want.
Then liquify pen(or mesh transform). Doesn't look that good here since I'm literally just trying it.

I've often heard this advice as "split bigger task into its parts" but applying it literally just makes the task overwhelming instead.
Specifying it as "batch similar parts" definitely helps.
Now to figure out the kind of batching works for me…
