Rangatahi Youth-Led Dissemination Campaign for Cocreated Eating and Well-Being Guidelines: Process and Pilot Implementation Evaluation Background: Youth should be partners in the development and dissemination of health information created for their demographic. The Manaora Rangatahi (youth) Guidelines comprise 10 eating and 10 wellbeing messages that were cocreated with rangatahi Māori (Māori youth) in Hawke’s Bay, New Zealand, and then disseminated through a digital media campaign. Objective: This study aimed to present the process evaluation of the co-design and implementation of the pilot digital media dissemination campaign of the Manaora Rangatahi Guidelines. Methods: The 17 rangatahi who were involved in the cocreation of the guidelines codeveloped a dissemination plan, filmed video clips for each of the 20 messages, and supported the 20-week digital media campaign. The codevelopment process over 4 wānanga (workshops) is described and critiqued, and the implementation was assessed using the data analytics from Instagram and TikTok, the main social media platforms for dissemination. The rangatahi participated in a short postcampaign review survey. Results: The dissemination plan included a website for the messages, a campaign name and logo, apparel with messaging and QR codes, and the design of a digital media campaign featuring the participating rangatahi. Video clips of each of the guideline messages featuring the rangatahi were professionally developed with support from 3 Māori influencers and a video production company. The 10 phases of the campaign involved the release of 2 messages (one eating and one wellbeing) every fortnight over 20 weeks. Instagram and TikTok analytics showed that the campaign achieved >1.48 million impressions and >19,000 engagement actions (eg, likes, comments, and sharing). The mean engagement rate (Instagram 6.2%, TikTok 1.2%) was greater than or similar to the platforms’ medians across all industries (0.36% and 1.73%, respectively). Various paid promotion strategies boosted the number of impressions, and paying one of the influencers to promote the messages in phase 8 created a more than 10-fold increase in impressions on Instagram. The estimated cost of the overall campaign was NZ $125,000 (US $72,500). The majority (about 60%) of rangatahi felt the campaign was successful and engaging. Conclusions: Rangatahi have expert knowledge in how to disseminate messages to their peers. They successfully co-designed and pilot-tested the implementation of a low-cost digital media campaign using peer-to-peer messaging and videos, which achieved substantial reach. The dissemination reach was good, but was significantly influenced by paid promotions. The cost per thousand impressions was equivalent to or better than much larger government-funded health promotion social media campaigns targeting youth. The development and dissemination of eating and well-being messages aimed at youth should ideally involve partnering with the target audience to enhance the relevance and reach of the messages. A well-funded dissemination campaign with paid promotions through influencers and digital platforms could expect to achieve substantial awareness of the messages.

Contextual Barriers to Health Information Systems Optimization in Underserved Communities in Kenya: Qualitative Study Informed by Frugal Innovation and Information and Communication Technologies for Development Background: Health information systems (HISs) are essential for strengthening health systems in underserved areas. However, many HISs in Africa are still in the early stages of implementation, and existing systems often suffer from imbalances in data availability. Their optimization is faced with various challenges, including limited resources, which restricts their scalability. Objective: The aim of this study is to identify contextual barriers that hinder the optimization of HIS in African underserved settings. Specifically, the study adopts the lens of frugal innovation (FI) and information and communication technologies for development (ICT4D) to explore ways to enhance the quality of health care delivery for low-income populations. Methods: A qualitative research approach involving 32 participants was used. The study was guided by the central theme: contextual barriers and challenges hindering the optimization of HISs. Results: Four major thematic categories emerged from the data: HIS contextualization, health system factors, service provider issues, and HIS integration. The findings offer valuable insights that can contribute to transforming HISs in underserved settings and improving health care quality. Conclusions: The findings reflect stakeholder experiences in underserved communities in Nairobi, Kenya, and may be transferable to similar settings, subject to local governance, resources, and workflows. Despite the transformative potential of HISs in low- and middle-income countries, progress remains limited due to poor digital infrastructure and contextual barriers resulting in minimal impact from capital-intensive digital health investments and persistent data challenges. Using FI and ICT4D lenses, 4 key barriers were identified: health system, HIS contextualization, HIS integration, and HIS service provider. Rethinking HIS strategies through FI and ICT4D can enable affordable and sustainable, user-centered solutions. Future research should test scalability, sustainability, and interoperability impact in diverse settings.

TikTok as a Platform for Patient Education and Health Information in Rare Genetic Diseases: Cross-Sectional Study Background: Rare genetic diseases pose significant diagnostic and therapeutic challenges, often leading to delayed diagnoses, misinformation, and patient isolation. Social media platforms have emerged as prominent spaces for health information dissemination and community building among patients with rare diseases. Objective: This study aimed to evaluate the role of TikTok videos in patient education, community engagement, and information quality related to 5 rare genetic conditions: Ehlers-Danlos syndrome, Marfan syndrome, cystic fibrosis, Wilson disease, and Gaucher disease. Methods: A cross-sectional analysis was conducted on 184 TikTok videos identified via disease-specific hashtags. Included videos were 15 seconds to 4 minutes long and directly discussed the target diseases. Advertisements, promotional content, and product marketing were excluded. Videos were categorized by creator type: physicians, medical professionals, patients, influencers, nonprofit organizations, and others. Content quality was assessed using the Global Quality Scale (GQS) and a modified DISCERN tool (mDISCERN). Engagement metrics (views, likes, and shares) were recorded. Kruskal-Wallis and chi-square tests evaluated differences across creator categories. Results: Of the 184 TikTok videos, 88 (47.8%) were created by patients or family members; 31 (16.8%) by influencers, 24 (13.0%) by physicians, 17 (9.2%) by nonprofit organizations, 15 (8.2%) by general users, and 9 (4.9%) by others. Collectively, the videos amassed more than 123 million views. Influencer-generated content accounted for the highest cumulative view count, totaling approximately 60.9 million views. Content produced by medical professionals and physicians demonstrated higher information quality, with mean GQS scores of 3.89 (SD 0.66) and 3.62 (SD 0.71) and mDISCERN scores of 3.11 (SD 0.58) and 3.21 (SD 0.65), respectively. In contrast, videos by influencers and patients exhibited lower quality scores (influencers: GQS mean 1.48, SD 0.60; mDISCERN mean 1.42, SD 0.55; patients: GQS mean 1.57, SD 0.58; mDISCERN mean 1.38, SD 0.52). For Ehlers-Danlos syndrome (n=40 videos, 21.7%), Wilson disease (n=40 videos, 21.7%), and cystic fibrosis (n=34 videos, 18.5%), significant differences in quality scores among creator types were observed (
