The study suggests doctors should test for HTRA1 in older patients when standard tests don't find a cause, since identifying the actual problem helps determine the right treatment. #renalpath #nephsky #pathsky

This new approach more accurately reflects what we know today. Better clarity on who's truly at risk means better decisions for patients and matters for clinical trial design and future treatments currently in development. #renalpath

Figure 1 Light and electron microscopy reveal epithelial cell vacuolation in acute tubular injury. (a) Light microscopy shows focal acute tubular injury at the corticomedullary junction, with significant swelling of tubular epithelial cells caused by optically transparent vacuoles. Hematoxylin and eosin staining. Bar = 50 μm. (b) The vacuoles contain periodic acid–Schiff–positive material. Bar = 50 μm. (c) Electron microscopy of paraffin-fixed tissue reveals the outline of tubular epithelial cell membranes, partial cell nuclei, and degenerated organelles, with obvious intracellular cavitation. Bar = 10 μm. To optimize viewing of this image, please see the online version of this article at www.kidney-international.org.

There were also mesangial IgA deposits (Figure 3), which likely explain the patient's hematuria and proteinuria, since there was very little glomerular amyloid (Figure 4).

The stain is a Congo Red viewed under fluorescent light with a red filter. It highlights interstitial amyloid deposits, which is the predominant pattern of deposition in LECT2 amyloidosis (confirmed by IHC stain, Figure 2).

Proteomic Profiling of Complement Components in Glomerular Disease - PubMed The complement system plays a central role in glomerular disease development and resolution. Currently, renal biopsies assess the presence of complement through limited immunofluorescence stains (C3 and C1q). With more than 50 total proteins and fragments involved in the complement cascade, this met …

We're excited to share a new research publication from @aaronstorey.bsky.social, @glombandit.bsky.social, and Dr. Chris Larsen titled, "Proteomic Profiling of Complement Components in Glomerular Disease."

Figure 1 Hematoxylin andeosin–stainedrenal biopsy(original magnification ×40) of the patient showingtubulointerstitial nephritis, characterized by a mononuclear inflammatory infiltrate with clusters ofmacrophages containing round intracytoplasmic structures (highlighted by the black circle). The Grocott-stained specimen (see Supplementary Figure S1) revealed intracytoplasmic round structures consistent with Histoplasma capsulatum yeasts. These histologic features supported the diagnosis of interstitial nephritis by Histoplasma spp. (see Supplementary Figure S1 for the differential diagnosis discussion).

Supplementary Figure S1. Grocott-stained specimen demonstrating intracytoplasmic round structures consistent with Histoplasma capsulatum yeasts (indicated by the red arrow). The differential diagnosis with other fungi (such as Cryptococcus, Candida, Blastomyces, and Paracoccidioides) and other intracellular pathogens (such as Toxoplasma, Leishmania, and Trypanosoma) was made based on the following histopathologic features: small size (2–5 μm), round shape, homogeneous intracellular distribution, silver positivity, absence of kinetoplasts, and negative mucicarmine staining—all of which supported the diagnosis of histoplasmosis.

Polymer Embolization From Minimally Invasive Interventions Many of the tools used in interventional nephrology, such as glidewires and sheaths, are coated with a hydrophilic polymer to increase their lubricity…

The PAS-stain picture depicts gray and basophilic amorphous material within glomerular capillaries, consistent with polymer emboli. The patient had undergone renal transplant artery angioplasty a few months earlier.
