Histoplasmosis remains an important cause of AIDS-related deaths in many regions
Histoplasma antigen tests are now cheap, accessible, and can be done at POC
Are such assays ready to be used for routine screening of patients with advanced HIV?
We have thoughts 🤔

I go around saying that tissue repair and tumorigenesis are two faces of the same coin. @ejvillablanca.bsky.social &co now show that this is not always the case and the two processes can be "unlinked" in the gut! @natureportfolio.bsky.social

Non-sputum, blood fingerprick triage diagnostic tests for pulmonary TB!
Nice work @jaynesutherlandtb.bsky.social. As you state there are still groups to verify this, but looks promising. Look forward to reading in detail.

agree! I need to detox actually, maybe phone intermittent fasting? 12-8 only (except on calls). I can feel the withdrawal effects already..
parents --> rearranging childcare and work to stay by bedside 'in case infection' to complete Abx and society, NHS cost, burden of AMR.
Lastly, many say, does it matter? Antibiotics are cheap and well tolerated? Think issue is for baby, Family and society. Baby- dysbiosis --> higher chance NEC, more sepsis, longer in hospital..
Our approach is host immune profiling with paired metagenomics with colleagues at Royal London - will report back!
..we know you can get local infection without bacteremia and not all organisms can be cultured in the first space! Think solution is innovation of better rule out tests at 24-36 hours to assist decisions on de-escalation, ie more objectivity.
trends in CRP and how baby doing....but still we often continue for a minimum of five days- why? Because it's nuanced- neonatologists worry about culture sensitivity given low blood volumes from neonates and possiblity false negatives (in theory),
Any stewardship here would require new very very good point of care tests, and even then, may not be sufficient to exceed threshold of clinician gut instinct. For stewardship, 24-36 hours seems sweet spot when you have had the opportunity to take a view, considering culture results..
My two cents (or pence)..important topic which requires nuanced approach. Very hard (imo) to affect change at point of starting empirical Abx. If you have a 28 weeker in front of you with signs, difficult to rule out sepsis and you know mortality very high if un/partially treated.
