trends in CRP and how baby doing....but still we often continue for a minimum of five days- why? Because it's nuanced- neonatologists worry about culture sensitivity given low blood volumes from neonates and possiblity false negatives (in theory),

Any stewardship here would require new very very good point of care tests, and even then, may not be sufficient to exceed threshold of clinician gut instinct. For stewardship, 24-36 hours seems sweet spot when you have had the opportunity to take a view, considering culture results..

My two cents (or pence)..important topic which requires nuanced approach. Very hard (imo) to affect change at point of starting empirical Abx. If you have a 28 weeker in front of you with signs, difficult to rule out sepsis and you know mortality very high if un/partially treated.
