• Cancer survivorship is increasing, thanks to early detection, more effective therapies, and supportive care, but sequelae of treatment may occur in the long term in these patients. • Chemotherapy (with/without immunotherapy/radiotherapy) may induce neurotoxic effects on the gastrointestinal innervation. • Chemotherapy-induced enteric neuropathy (CIEN) and, probably, neurotoxicity affecting the extrinsic innervation of the gut and the brain underlie the frequent and impactful gut dysfunctions and brain–gut axis (BGA) disorders occurring in the long term in cancer survivors.
