Smart Kiosk for Nutritional Management of People With Diabetes in Underserved Communities: Development and Technical Evaluation Background: Diabetes is a chronic disease with a high global prevalence, increasing from 200 million people in 1990 to 830 million in 2022, with a higher burden in low- and middle-income regions and high mortality in Mexico and Veracruz. These inequalities limit access to treatment and nutritional education, requiring technological solutions such as interactive kiosks based on artificial intelligence (#AI) (AI) that contribute to the nutritional management of people with diabetes in marginalized communities. Objective: This study aimed to design and evaluate an interactive kiosk based on AI that generates culturally relevant and personalized meal plans for people with diabetes in marginalized communities. Methods: A low-cost prototype was developed, with a database of local foods and a multilayer perceptron trained with synthetic data based on national clinical guidelines. Performance was tested through an experimental evaluation that measured (1) the accuracy of nutritional recommendations compared with ideal meal plans (accuracy, precision, sensitivity, and -score); (2) performance, measured by recording response time with 1 to 50 simultaneous requests; and (3) #usability, assessed using heuristic evaluation and the System #usability Scale (SUS). Results: The smart kiosk was experimentally evaluated in three dimensions: nutritional recommendations, system efficiency, and #usability. The model achieved AI metrics of 87.3% overall accuracy, 90.5% precision, 92.1% sensitivity, and 91.3% -score. The average response time was 2.36 (SD 0.24) seconds in all load tests. A maximum time of 4 seconds was obtained in the simulation of 50 concurrent users. In the #usability evaluation, an average score of 89 (SD 2.89) out of 100 was obtained on the SUS, which is considered excellent, along with a success rate of 98.3%. Conclusions: The AI-based kiosk demonstrated technical #feasibility, adequate performance, and satisfactory #usability. Its ability to operate without the need for internet and its low cost make it an equitable option for diabetes self-management and a replicable model in public health.

Personalized Diabetes Treatment Support Using Large Language Models Fine-Tuned on Electronic Health Records: Development and Evaluation Study Background: Effective diabetes management requires individualized treatment strategies tailored to patients’ clinical characteristics. With recent advances in artificial intelligence (#AI), large language models (LLMs) offer new opportunities to enhance clinical decision support, particularly in generating personalized recommendations. Objective: This study aimed to develop and evaluate an LLM-based outpatient treatment support system for diabetes and examine its potential value in routine clinical decision-making. Methods: Three compact LLMs (LLaMA-3.1-8B, Qwen3-8B, and GLM4-9B) were fine-tuned on de-identified outpatient electronic health records using a parameter-efficient LoRA approach. The optimized models were embedded into a prototype hospital information system via a retrieval-augmented generation (RAG) framework to generate individualized treatment recommendations, laboratory test suggestions, and medication prompts based on demographic and clinical data. Results: Among the models evaluated, the fine-tuned GLM4-9B demonstrated the strongest performance, producing clinically reasonable treatment plans, appropriate laboratory recommendations, and medication suggestions. It achieved a BLEU-4 score of 67.93 ± 2.74 and ROUGE scores of 44.30 ± 3.91 (ROUGE-1), 27.34 ± 1.85 (ROUGE-2), and 37.67 ± 2.88 (ROUGE-L). Conclusions: The fine-tuned GLM4-9B shows strong potential as a clinical decision support tool for personalized diabetes care. It can provide reference recommendations that may improve clinician efficiency and support decision quality. Future work should focus on enhancing medication guidance, expanding data sources, and improving adaptability in cases involving complex comorbidities.

A Culturally Tailored Diabetes Self-Management Education Program With Mobile Health Integration for Chinese Americans With Type 2 Diabetes: Development and Pilot Evaluation Study Background: Although progress has been made in improving the efficacy of Diabetes Self-Management Education (DSME) programs, there remains a dearth of research on culturally adapted, evidence-based DSMEs for Chinese Americans (CAs) with type 2 diabetes. Objective: Through collaborative partnerships with 2 large community recreation centers and the AHMC Hospital Network in San Gabriel Valley, California, we developed and pilot-tested a culturally tailored DSME program with integrated mobile health (mHealth) technology, entitled Culturally Appropriate Strategies for Chinese Americans with Diabetes (CASCADe). Methods: The CASCADe program utilized a combined, theoretically driven, and community-participatory approach and was developed based on information gleaned from focus groups, semistructured interviews, and a questionnaire survey conducted among CA patients with diabetes, physicians, and nurses, as well as from extensive literature reviews of evidence-based program curricula. A single-group pre- and posttest design with a 3-month study period was then employed to assess the program’s preliminary efficacy. The study protocols were registered on ClinicalTrials.gov. Results: The CASCADe program consisted of (1) a home visit in the first month for training in monitoring device use and WeChat app (a mobile instant-messaging platform widely used in the Chinese population) usage, as well as for acquiring family support; (2) 8 weekly sessions over the following 2 months, delivered in a combined format of group classes, games, group exercises, videos, and discussions; and (3) WeChat follow-up involving education tips, monitoring data summaries, and group discussions after each of the 8 weekly sessions. Topics covered in the weekly sessions included recognition of diabetes and its complications, risk factors, nutrition knowledge, dietary practices, exercise, behavioral self-monitoring, medication adherence, and stress management. The monitoring system used a smartphone to coordinate cloud-based data transmission from a set of wireless devices to capture daily monitoring data on physical activity, body weight, blood pressure, and blood glucose levels. Behavioral self-monitoring was further facilitated by the WeChat app, which provided daily messages related to the diabetes education curriculum; weekly summary reports of monitoring data; feedback; bidirectional 1-on-1 communication between intervention providers and participants; and group discussions among participants regarding readings and the implications of monitoring results. The pre- and postcomparison from the 3-month pilot trial showed a significant reduction in glycated hemoglobin (HbA1c; 7.48 vs 7.09, P=.03), with all but 1 participant demonstrating a reduction and 7 out of 12 (58%) achieving a >0.5 decrease in HbA1c. Significant improvements were also observed in self-efficacy in diabetes management (6.59 vs 8.01, P=.003), quality of life (3.21 vs 3.69, P=.005), and stress-coping skills (3.18 vs 3.74, P=.01) at 3 months after baseline among CA patients with type 2 diabetes. Conclusions: Our pilot study demonstrated the #feasibility of implementing the CASCADe program among CAs to improve diabetes self-management skills and yielded promising results, warranting further evaluation in a larger randomized trial. Developing a Customizable Texting Intervention for Diabetes Self-Management: Participatory Design Approach Background: Uncontrolled diabetes contributes to serious comorbidities and mortality. Effective self-management can improve outcomes, though barriers such as limited education and support often prevent patients from engaging in such behaviors. Automated texting systems show promise to deliver diabetes self-management education as they are accessible and scalable. Furthermore, customizing these systems may further enhance patient engagement compared to standard, one-size-fits-all approaches. However, such customization is more resource-intensive, and it remains unclear whether the added effort meaningfully enhances diabetes self-management and outcomes. Objective: This study aimed to describe the development of 2 versions of an automated texting system intervention for diabetes self-management: (1) a standard, education-only intervention (Diabetes Self-Management Support; DSMS) and (2) an interactive, customizable intervention (Diabetes Self-Management Support + Interactive and Customizable Messages; DSMS+). Methods: Two versions of an automated texting system intervention were developed using a participatory design approach that incorporated input from veterans and expert clinicians. Message content was refined through feedback from a multidisciplinary team, veteran coinvestigators, national surveys, interviews, clinical expert panel reviews, and beta testing. Surveys were mailed to 1000 potential participants, oversampling rural, low-income, minority, and female participants. Respondents rated message relevance and provided preferences for content, timing, and frequency. Interviews provided customization preferences. A clinical expert panel reviewed all messages for safety and appropriateness. Beta testing informed final refinements. Results: Ninety-two surveys were completed (9.2% response rate). Respondents rated 62% of the messages as personally relevant and 61% confidence-enhancing. Interviews with 23 respondents revealed a preference for 1-2 texts per day, emphasizing topics such as healthy eating and weight management. The clinical expert panel reviewed 536 messages, flagging 81 for revision. Beta testing confirmed #feasibility and informed refinements to clarity and timing. The 2 resulting interventions were built in the US Department of Veterans Affairs’ automated texting system, Annie. Conclusions: Two #TextMessaging #mhealth interventions, DSMS and DSMS+, were developed to support diabetes self-management among US veterans. DSMS delivers standard educational content, while DSMS+ incorporates interactive features and personalization. The subsequent clinical trial will assess whether customization enhances engagement and improves diabetes outcomes, providing insights into the potential of tailored mobile health interventions for chronic disease management.

The Impact of a Digital Peer-Supported App on Daily Steps and Lifestyle Changes Among Individuals With Prediabetes and Early-Stage Type 2 Diabetes: Prospective, Nonrandomized Controlled Trial Background: Physical activity is a simple, low-risk intervention that could be integrated into daily life to improve glycemic control in individuals with prediabetes and early-stage type 2 diabetes mellitus (T2DM). However, maintaining physical activity remains challenging, even when its benefits are well understood. Although digital peer support has the potential to promote and maintain physical activity, its effectiveness has not yet been sufficiently established. Objective: This study examined the impact of a digital peer-supported app on daily step goal achievement and average daily step counts among individuals with prediabetes and early-stage T2DM. Methods: This 3-month, prospective, nonrandomized controlled trial recruited participants aged 40-79 years with prediabetes or early-stage T2DM. The participants were divided into a digital peer-supported app group and a control group. The digital peer-supported app group tracked their daily steps, shared their progress with small peer groups, and received real-time feedback and support within the app. The control group tracked their steps individually using a pedometer. The primary outcome was the achievement rate of daily step goals. Secondary outcomes included the average daily step count, BMI, glycosylated hemoglobin A1c level, blood pressure, and self-reported lifestyle behaviors. Results: A total of 32 participants (digital peer-supported app group: n=18 and control group: n=14) completed the study. The digital peer-supported app group reported a significantly higher median daily step goal achievement rate (57.2%, IQR 32.2%-90% vs 26.7%, IQR 10%-64.4%; P=.04) and daily step count (6854, IQR 4846-10388 steps vs 3946, IQR 3176-6832 steps; P
