Text saying: A New Question

Racial bias in pulse oximetry is a long-described, technologically solvable, but unsolved threat to patient safety. It is consequential for patients and health systems.

What are the institutions, organizations, and policies that have locked us into the current mediocre technology?

FDA Guidance on Pulse Oximetry Testing Among Diverse Individuals This study examines temporal trends in US Food and Drug Administration clearance summaries for pulse oximeters that report information about race and ethnicity and/or skin color or tone.

Let us hope—and expect—that our industry colleagues do a better job of implementing this #PulseOx guidance than occurred with the 2013 guidance:

Fixing pulse oximeters requires federal might and possible legal action, researchers say The devices remain unreliable in darker skinned people. Researchers say more guidance, enforcement and even legal action may be needed to spur improvement.

“More is needed to help develop and bring to market newer [ #PulseOx ] devices that don’t have the same problems,” said Shachar. “Once we have alternatives, more thinking needs to be done about whether we can remove the older devices”

Six of the 11 POX tested met the FDA criteria of ARMS < 3% between the SaO2 range of 70 - 100% (Table 2). The remaining five of the 11 POX had an ARMS > 3%. Nine of 11 POX met ISO Standard criteria for ARMS < 4%. Individual oximeter performance data are presented in Table 2. The Nonin Onyx Vantage and Masimo MightySat had the best ARMS at 1·9% and 2·1%, respectively. Nine of the 11 POX demonstrated worse ARMS in study subjects categorized at the lowest third of ITA values (dark), compared with the highest third of ITA values (light), measured at the dorsal DP (Table 2). As SaO2 levels decreased, mean bias (SpO2 - SaO2) increased for ten of the 11 devices (Figure 3).
