Sustaining a Promising Clinical Practice in High-Turnover Rural Environments Through the Geriatric Referral Navigator Role: Qualitative Case Study Background: Sustaining evidence-based health care programs is challenging, especially in clinical settings with high staff turnover. GRECC Connect is an evidence-based telemedicine service that provides geriatric specialty care to rural patients through a hub-and-spoke model between US Department of Veterans Affairs medical center “hubs” and community-based outpatient clinic (CBOC) “spokes.” Four geographically diverse GRECC Connect hub sites (of 19 total) volunteered to implement a “geriatric referral navigator” role to address the challenge of sustaining use of this program in rural CBOCs. Objective: This study aimed to understand how a health care program such as GRECC Connect can design and implement a navigator role to sustain use of its services. Methods: We conducted a longitudinal qualitative study using a case study approach. Participants were GRECC Connect hub site staff, including program directors, coordinators, and analysts from a range of disciplines, including clinical pharmacists, geriatricians, and social workers. Over one year, we conducted 31 qualitative interviews (5 to 9 “periodic reflection” meetings and 2 semistructured interviews at each of the 4 sites) focused on key tasks, skills, and characteristics of the geriatric referral navigator role along with perceptions about the role’s value. Each of the group interviews involved 2 to 4 staff members (N=10). We conducted directed content analysis using a rapid analytic approach and then shared deidentified preliminary findings at a national GRECC Connect online meeting where staff from all 19 hub sites (n=40) reflected on the role. We summarized and compared the perspectives shared during that meeting with the data we collected and analyzed from the 4 participating hub sites. Results: Key navigator tasks included building relationships, providing education, monitoring and troubleshooting logistical and technological issues within and across CBOCs, and evaluating the appropriateness of referrals. While professional backgrounds varied, navigator traits deemed essential for success included being flexible, creative, and a problem solver with deep institutional knowledge. The time needed to conduct navigator tasks—between 15% and 70% of the time required of a full-time employee—was substantial. The navigator role resembles several roles described in the literature that are meant to support program implementation and service use, including internal champions, external facilitators, and clinical navigators. Navigator tasks reflected a combination of known “semivisible” implementation strategies that hub site staff considered necessary not only for initially implementing the program but also for sustaining use of GRECC Connect in rural clinical environments with high staff turnover. Conclusions: The geriatric referral navigator role encompasses a broad array of recognized implementation strategies. It is critical to invest in supporting the types of tasks and strategies implemented by the geriatric referral navigator to maintain promising practices over time, where appropriate, to avoid the costs and burdens of implementing new, similar programs in the future.

Acceptance of Digital Technology Among Nursing Staff in Geriatric Long-Term Care: Systematic Review Background: Digital technologies are increasingly being introduced into the healthcare system and in settings like hospitals and geriatric long-term care (LTC) facilities, offering potential benefits such as improved care quality, reduced workload or enhanced documentation processes. However, the success of these technologies depends also on their acceptance by the primary users, the #nursing staff, especially in the field of geriatric LTC, where the care employees are more involved in basic care of the elderly. Objective: This work synthesizes subsequent empirical studies that have explored the acceptance of digital technologies by #nursing staff in geriatric LTC settings, building upon the foundational work by Yu et al. which was published in 2009 [1]. The goal is to highlight the extent to which this topic has been addressed in the literature, to identify influencing factors that have emerged in the literature over the past years regarding the acceptance of digital technologies in geriatric LTC facilities and to highlight the lack of empirical studies in the LTC-field. Methods: A systematic literature review was conducted following PRISMA 2020 guidelines. The SPIDER framework was used for eligibility criteria. Databases searched included PubMed, ACM Digital Library, Web of Science and the Health Administration Database (ProQuest). Studies were included if they empirically examined the acceptance of digital technologies by #nursing staff in geriatric LTC settings. Two reviewers independently screened the studies, extracted data and assessed methodological quality using the CASP (Critical Appraisal Skills Programme) checklist. Results: A total of three studies met the criteria, highlighting a gap in research on this topic. Although limited in number, these studies offer initial insights and provide a basis for further investigation. The studies applied cross-sectional quantitative designs, highlighted critical determinants of technology acceptance, including perceived usefulness, ease of use, digital competence and organizational support. The studies involved a total of n=1,019 participants from Germany, Australia and The Netherlands. Barriers included lack of user involvement, lack of training, poor system design and demographic differences in digital affinity. Conclusions: This review underscores the insufficient scientific attention to digital technology acceptance among LTC #nursing staff. Nevertheless, the results show that the acceptance of digital technologies by #nursing staff in geriatric LTC settings is triggered by a constellation of individual factors, like digital competence and perceived relevance of technology and organizational factors like access to training and involvement of staff in the implementation process. However, to ensure sustainable digital transformation in geriatric LTC, further scientific evaluations to this specific care setting are urgently needed.

Evaluation of an AI-Based Clinical Decision Support System for Perioperative Care of Older Patients: Ethical Analysis of Focus Groups With Older Adults Background: The development and introduction of an AI-based Clinical Decision Support System (CDSS) in surgical departments within the SURGE-Ahead project responds to the increasing aging population. Thereby, digital geriatric co-management with an evidence-based evaluation of the patient’s health condition and corresponding medical recommendations is aimed to improve the perioperative geriatric patient care. Objective: The use of an AI-based CDSS in patient care raises ethical challenges. The collection of opinions, expectations and concerns of older people as potential geriatric patients on the CDSS enables the identification of ethical chances, concerns and limitations that arise after implementing the technical device in hospitals. Methods: In total, five focus groups with participants aged 65 years and above were conducted. The transcripts were evaluated according to the qualitative content analysis and ethically analyzed: First, an inductive formation of categories was implemented, followed by a thematic classification of the participants’ statements. Thereby, we disproved technical understanding to impact the older people’s opinions. Results: Ethical chances and concerns were detected: The diagnosis and therapy could speed up, changes in the patient-AI-physician-interaction could improve medical treatment and enhance the coordination in hospitals. However, the quality of the CDSS depends on an adequate data basis and cyber security. A habituation effect and loss of a second medical option could develop, and the severity of an illness could be considered as an impact factor on the patient’s attitude towards medical suggestions. The risk of overdiagnosis and overtherapy was discussed controversially, and the range of therapy options could be influenced by interests and finances. Saving time resources would remain challenging, medical skills could decrease and the extent of the patient’s hospital stay could be affected. Conclusions: To respond to the ethical challenges, we recommend a time-sufficient use and emphasize an individual revision of the CDSS’s results. Furthermore, we suggest a limitation of private financial sponsoring.

Socioculturally Appropriate Internet-Based Geriatric Care Model for Older Adults Living With HIV: Experience-Based Co-Design Approach Background: Older adults living with HIV face challenges accessing regular geriatric care, and while virtual care services could offer a solution, they may come with limitations. Objective: This study aimed to co-design a culturally appropriate virtual care model tailored to older adults’ needs using the experience-based co-design methodology. Methods: We used a qualitative, experience-based co-design approach with 19 older adults living with HIV. The process involved 3 phases: identifying needs through interviews and questionnaires, codeveloping a care model prototype through focus groups and a workshop, and refining the model using feedback from a world café format. Data were analyzed using thematic content analysis. Results: The co-design process led to a virtual care model prototype that directly addressed participants’ key needs. These included personalized communication methods, simplified technology interfaces for easier access, and culturally responsive care practices. Participants emphasized the importance of privacy in virtual consultations, flexible scheduling to accommodate health fluctuations, and ongoing support for managing both HIV and aging-related conditions. Their feedback shaped a model designed to bridge service gaps, offering a more inclusive, accessible, and patient-centered approach to virtual geriatric care. Conclusions: This study co-designed a potential virtual geriatric care model grounded in the experiences of older adults living with HIV. By integrating participants’ insights throughout the design process, the model offers a promising approach to improving care for this vulnerable population. Future directions for research to test this model are proposed.
