Partial REBOA looks like a gentler way to buy time in hemorrhagic shock.
In pigs, partial occlusion preserved mesenteric flow and jejunal microcirculation vs total REBOA, with better pH/lactate trends. Tracked continuously with #OxyFlo Pro laser Doppler (jejunal mucosa + serosa).

Our study of 601 TCA cases shows:
✅ 40% survival with RT within 10 min for cardiac tamponade, 21% for tamponade overall.
🚨 1.6% survival for exsanguination-induced TCA - STOPPING the bleeding before arrest is critical! #REBOA
⏳ No survivors beyond 15 min (tamponade) or 5 min (exsanguination).

And a last one: out of the 16 participating sites, 3 included more than 2 pts per year. At a 1:1 randomisation, more than half the pts in the trial were recruited in sites with <1 #REBOA / year.
I have immense respect for the ppl who pulled off this trial, but I don't think it kills REBOA.
Yet another issue: upper IQR for systolic blood pressure is 115mmHg, meaning that 25% of pts in the control condition had a systolic pressure higher than that. Not sure how one qualifies for a trial of #REBOA with such a pressure.
