Comparison and Validation of Actigraphy Algorithms Using a Large Community Dataset: Algorithm Validation Study Background: For decades, the measurement of sleep and wake has relied upon watch-based Actigraphy as an alternative to expensive, obtrusive, clinical monitoring. To date, we have relied upon a handful of algorithms to score actigraphy data as sleep or wake. However, these algorithms have largely been tested and validated with only small samples of young healthy individuals. Objective: To address this issue, this study established the accuracy and agreement of conventional and traditional actigraphy algorithms against polysomnography the clinical standard using the diverse MESA sleep dataset. As a secondary objective, we examined algorithm and polysomnography agreement for key sleep metrics including total sleep time (TST), sleep efficiency (SE), and wake after sleep onset (WASO). Methods: We assessed 5 well established algorithms including Cole-Kripke, UCSD scoring, Kripke 2010, Philips-Respironics, and Sadeh, with and without rescoring across 1440 individuals (Mage=69.36+/-8.97) from the MESA sleep dataset. We conducted epoch-by-epoch comparison assessing accuracy, confusion matrix analyses, Receiver Operator Characteristic Curves, Area Under the Curve, and Bland Altman analyses for agreement. Results: Primary results indicated all algorithms demonstrated accuracy between 78%-80% with the highest accuracy by the Kripke 2010 (80%) algorithm and closely by Cole-Kripke (80%) and Philips-Respironics (80-79%) algorithms. In addition, moderate Cohens kappa agreement and moderate positive Matthews correlations were demonstrated by all algorithms. Further, all algorithms demonstrated significant mean difference across sleep metrics. Conclusions: The findings of this study establish that these traditional actigraphy algorithms can, with high accuracy, detect sleep and wake in large diverse population samples, including older adults, or populations at risk of health conditions. However, these algorithms may carry difficulty for precise assessment of sleep metrics especially in cases of sleep disorders or irregular sleep.

Sleep and Activity Patterns as Transdiagnostic Behavioral Biomarkers in Psychiatry: Longitudinal Observational Study From the DeeP-DD Study Background: Despite widespread use of symptom rating scales in psychiatry, these tools are limited by reliance on self-report, infrequent administration, and lack of predictive power. This constrains clinicians’ ability to monitor illness trajectories or anticipate adverse outcomes like relapse. Actigraphy, a passive wearable-based method for measuring sleep and physical activity, offers objective, high-resolution behavioral data that may better reflect symptom fluctuations. Prior research has shown associations between actigraphy features and mood or psychosis symptoms, but most studies have focused on narrow diagnostic groups or fixed time windows, limiting clinical translation. Objective: To examine whether actigraphy-derived sleep and activity features correlate with psychiatric symptom severity in a transdiagnostic psychiatric sample, and to identify which features are most clinically relevant across multiple temporal resolutions. Methods: We present a #feasibility case series study analyzing preliminary data from eight outpatients (ages 18–52) enrolled in the Deep Phenotyping and Digitalization at Douglas (DeeP-DD) study, a prospective transdiagnostic study of digital phenotyping. Participants wore wrist-based actigraphy devices (GENEActiv) for up to five months. Symptom severity was measured using a variety of self- and clinician-rated scales. We performed intra-individual Spearman correlations and inter-individual repeated measures correlations across daily, weekly, monthly, and full-duration averages. Comparison of Sleep Features Across Smartphone Sensors, Actigraphy, and Diaries Among Young Adults: Longitudinal Observational Study Background: Poor sleep health is pervasive and contributes to long-lasting physical and psychological problems. As traditional sleep measurement can be burdensome, testing scalable and accessible sleep measurements is important. Objective: The aim of this study was to test whether sleep features obtained through a smartphone app are comparable to other modes of sleep measurement (i.e., daily diary, wearable actigraphy). Methods: College students (n=29) answered daily questions about their sleep and provided smartphone accelerometer data using the Effortless Assessment Research System (EARS) application for one week. Analyses compared bedtime, risetime, and time-in-bed across diary and EARS. Wrist wearable actigraphy in 13 participants was used in supplementary analyses. Results: On average, EARS showed a high true positive rate (86.6%) and low false positive rate (4.0%) in identifying bedtimes and risetimes. There was no significant difference among diary, actigraphy, and EARS bedtime, risetime, and time-in-bed (Ps≥.069). Day-to-day sleep features were significantly correlated between among diary, actigraphy, and EARS (rs≥0.29, Ps≤.001), except EARS and actigraphy risetimes (r=0.29, P=.067). Conclusions: Smartphone-based sleep sensors show acceptable alignment with more established methods and may provide a feasible alternative to measuring daily sleep patterns in a scalable way. Future studies will require larger, diverse samples to corroborate findings of concordance among EARS, diary, and actigraphy data in other populations.
