It notes that after #liftoff, the fuel cutoff switches had been moved to the cutoff position. The switches were returned to the run positions, but too late.

We have heard more about every other incident every #nearmiss than we have heard about that plane that was sabotaged by a sad #pilot and crashed into a building.
