Identifying Undiagnosed High-Risk Suicidality Cases by Matching Patients With a Similar Comorbidity Burden: Retrospective Observational Study Background: Suicide is the second leading cause of death for children and adolescents aged 6 to 18 years. Pediatric suicidality is underreported, which poses significant challenges for effective intervention and prevention strategies. Identifying populations at risk of suicidality can provide critical benefits in terms of study cohort selection, prevalence estimation, and clinical resource allocation. Objective: This study sought to (1) measure the prevalence of mental health comorbidities in pediatric suicidality and (2) identify undiagnosed high-risk suicidality cases by matching them with patients with a similar mental health comorbidity burden. Methods: Electronic health record data from a large academic pediatric hospital in Boston, Massachusetts, were analyzed for patients aged 6 to 18 years presenting to the emergency department between June 1, 2016, and June 1, 2022. Suicidality cases were defined using () codes for 3 suicidality subtypes: suicidal ideation, self-harm, and suicide attempt. Comorbidities of suicidality were calculated as the conditional probability of code pairs. After multiple hypothesis corrections, statistically significant comorbidities and patient encounter demographics were input as covariates into a propensity score matching (PSM) model. The accuracy of the PSM model was validated against chart review by 2 independent subject matter experts. Results: In total, 2.9% (2638/90,980) of emergency department encounters met an –based case definition of suicidality during the study period. The prevalence of suicidality by subtype was 2.5% (2275/90,980) for ideation, 1.1% (1030/90,980) for self-harm, and 0.2% (177/90,980) for suicide attempt. Suicidality prevalence was more common for the female sex (1825/43,929, 4.2%) than for the male sex (813/47,045, 1.7%). Comorbidities of suicidality were statistically significant for 55 frequently co-occurring codes. Nearly half of these comorbidities (26/55, 47.3%) were not present in the , and nearly a quarter (12/55, 21.8%) consisted of codes for accidental rather than intentional self-harm. Increased probability of suicidality was observed for patients with personality disorder (84/190, 44.2%), gender dysphoria (143/333, 42.9%), bipolar disorder (162/448, 36.2%), depression (1791/5426, 33%), and schizophrenia spectrum disorders (133/411, 32.4%). On the basis of gold-standard chart review, 53.4% of propensity-matched noncases were unrecognized cases of suicidality. Conclusions: PSM using comorbidity profiles is an effective approach for identifying suicidality cases that lack codes for suicidality.

Autistic traits and suicidality in midlife and old age: investigating mediating effects of mental health and social connectedness - Nature Mental Health Nuzum et al. used cross-sectional data from the UK PROTECT study to evaluate the relationship between high autistic traits and suicidality in older adults.

Transgender youth who receive hormone therapy are less suicidal: study There's simply no denying that gender-affirming care is life-saving. Transgender youth who are able to access hormone replacement therapy are less likely to be suicidal, according to a report recently published in The Journal of Pediatrics. Researchers surveyed 432 patients ages 12 to 20 over 679 days, finding that "suicidality significantly declined from pretreatment to post-treatment" across all sexes, ages, and treatment durations. **Related:****The kids aren't alright: Depression and anxiety rates are skyrocketing among LGBTQ+ youth** "HT was associated with clinically meaningful reductions in suicidality over time, extending prior findings with a larger sample and longer follow-up," the report concludes. "These study findings provide clinical evidence supporting the mental health benefits of timely access to HT in this population." Hormone replacement therapy (HRT), which falls under gender-affirming care, involves a patient taking either feminizing (estrogen) or masculinizing (testosterone) hormones for the purpose of altering their appearance to better reflect their gender identity, typically in the form of an injection. Gender-affirming care for prepubescent youth primarily focuses on socially transitioning — changing their hair, clothing, or potentially going by a new name and pronouns. Only after many months being evaluated in talk therapy could a pubescent child demonstrating gender dysphoria be prescribed puberty blockers. When the patient is old enough, usually ages 16 to 17 per Planned Parenthood, they can then be prescribed hormones to replace those produced by their body. The American Medical Association, the American Psychiatric Association, the American Academy of Pediatrics, the American Academy of Child & Adolescent Psychiatry, the World Medical Association, and the World Health Organization all agree that gender-affirming care is evidence-based and medically necessary not just for adults but minors as well. **Related: Transgender and nonbinary youth whose pronouns are respected attempt suicide less: report ** A separate December report from The Trevor Project found that trans and nonbinary youth (ages 13 to 24) whose pronouns are respected were 31 percent less likely to attempt suicide in the past year than those whose pronouns are not respected.
