Thanks. Another problem for me is I vastly prefer #inpatient to #outpatient work, hard to find a #Geriatrics job that doesn’t involve #clinic. I prefer things like #consults or something like GEM (geriatrics evaluation and management) at VA, but I know being Goldilocks in this #economy is impossible

That is now exacerbated by the fact that #financial headwinds from the #BigUglyBill and tremendous #Medicaid cuts are putting strain on #primarycare and massively increasing #boarding in our #inpatient hospitals, and those that are #Medicare eligible are en masse saying "I'm out"
Yes, the #Medicare #waiver requires the #patient to have either been seen in an #emergencydepartment or coming from an #inpatient #hospital. We actually previously took a few patients directly from home into our (now defunct) HCAH program, but we weren't paid for those visits at all 🤦♀️
blue line is the utilization of said beds, number of #pediatric #inpatient days. Kentucky actually seems to be the only state that may have a surplus of #pediatric beds compared to how much it's using them.
KY is a weird state, deeply red but a bit more sane? than other red states

I feel ya. With our retirements in #primarycare #Geriatrics, there's definitely a need, but I've spent so much time on #hospitalist side I would struggle with that switch. I'd do it if I have to, but I definitely am more #inpatient -coded. Give me dobutamine gtt over #diabetes CGM management any day
Oh I completely agree. #Rheumatology was actually on my specialty short list when I was in residency, I loved the #inpatient consults, but as someone who anticipated being full-time clinical the #outpatient practices were absolutely brutal. Upwards of 40+ complex patients/day, chronic pain, etc
