Locomotive Syndrome Digital Therapeutics Provided via a Smartphone App: Proof-of-Concept Single-Group Trial Study Background: Individuals with locomotive syndrome (LS) have muscle weakness and reduced motor function due to musculoskeletal disorders that cause reduced mobility and physical function. In Japan, musculoskeletal disorders are the most common reason for requiring home support or nursing care, highlighting the need for preventing and ameliorating LS. Middle-aged and older adults sometimes encounter difficulty making a habit of exercise therapy (the mainstay of LS treatment). Objective: We investigated whether digital therapeutics (DTx) can prevent or ameliorate LS in middle-aged and older adults. Methods: We conducted a prospective, longitudinal, nonrandomized, single-group study of Japanese aged 40 years and older who were eligible for LS checkups (N=47). Each participant underwent an 8-week locomotion training intervention based on DTx supervised by medical staff. We objectively assessed the participants’ subjective and objective motor abilities and motor awareness with the Timed Up and Go (TUG) test, 25-question Geriatric Locomotive Function Scale (GLFS-25), and Behavioral Regulation in Exercise Questionnaire-3 at baseline (before the DTx), an interim point (4 wk after the DTx initiation), and a final evaluation (8 wk post-DTx initiation). We compared the scores of the 3 tests at the 3 time points as dependent variables in a 3-factor ANOVA with Bonferroni correction (significance defined as 0.05/3=0.0167). Results: No increase in amotivation to exercise or refusal to exercise was observed. Significant improvements at 8 weeks versus the baseline were observed in the TUG scores (baseline: 9.0, 95% CI 8.4‐9.6; 8 wk: 7.5, 95% CI 7.1‐8.0; =.001) and GLFS-25 results (baseline: 18.7, 95% CI 14.5‐22.8; 8 wk: 11.7, 95% CI 8.8‐14.7; =.004). The Behavioral Regulation in Exercise Questionnaire-3 and its subscale data did not differ significantly at any assessment time point. Conclusions: These results indicate that an 8-week locomotive training intervention using DTx significantly improved middle-aged and older adults’ TUG and GLFS-25 scores and will help prevent and ameliorate LS and establish better exercise habits among them. Safe Patient Handling Musculoskeletal Injury-Prevention Smartphone App for Community Health Care Workers: Mixed Methods #feasibility Study Background: Safe patient handling is critical for reducing musculoskeletal injuries among health care workers; yet, community health care workers often face barriers such as limited access to training and real-time resources. Objective: This study had three objectives: (1) provide detailed insights into the unmet needs of Island Health community health care workers with respect to safe patient handling resources and access to information, (2) translate those needs into a user-centered prototype of the Safe Patient Handling Musculoskeletal Injury-Prevention (SPH MSIP) smartphone app through an iterative co-design process, and (3) establish the acceptability and #feasibility of SPH MSIP app to support community health care workers’ safe patient handling practices using a mixed methods design. Methods: A 3-phase participatory study was conducted. Phase 1 identified unmet safe patient handling needs through participatory meetings with 6 community health care workers, aligning with objective 1. Phase 2 involved developing the SPH MSIP app using co-design methods, integrating user feedback to address challenges such as guidance for high-risk tasks and intuitive design, addressing objective 2. Phase 3 evaluated the app’s #feasibility and acceptability, aligning with objective 3. The study recruited 28 participants who used the app for one month. A single-group mixed methods design was used, incorporating quantitative metrics such as recruitment (≥50%), retention (≥75%), and satisfaction (mean score ≥4). Qualitative feedback was gathered through small-group interviews to understand #usability, usefulness, and integration into workflows. Results: In phase 1, community health care workers identified barriers, including limited safe patient handling, refresher training, and isolation during tasks. In phase 2, the app was developed to address these safe patient handling needs, incorporating features like scenario-specific guidance for high-risk tasks. In phase 3, the app exceeded success criteria for recruitment, retention, and satisfaction, with participants highlighting its usefulness, #usability, and adoption. Qualitative feedback emphasized the app’s practical value as a real-time resource, particularly its step-by-step guidance and user-friendly design. Conclusions: This study met its objectives, highlighting the SPH MSIP app’s potential to address community health care workers’ unmet safe patient handling needs and improve support in real time patient handling scenarios. While the findings suggest strong #feasibility and acceptability, future research should focus on large-scale, extended effectiveness trials to evaluate the app’s impact on reducing musculoskeletal injury rates and improving patient care outcomes. Convergent and Known-Groups Validity and Sensitivity to Change of the Virtual Performance Measure in Patients With Hip and Knee Osteoarthritis: Longitudinal Study Background: Subsequent to the #covid19 pandemic in 2020, a different approach to health care utilization was required to improve safety and efficiency. In the postpandemic era, virtual care and remote assessment of musculoskeletal conditions has become more common, and examining the accuracy of these remote encounters remains vital. In 2023, an innovative, video-based tool—the Virtual Performance Measure (VPM)—was introduced to assess the functional difficulties of patients with osteoarthritis of the knee joint. Further validation of this tool is warranted to expand its application longitudinally and in more diverse populations. Objective: This study examined the longitudinal validity of the VPM, a digitally based outcome tool, in patients with osteoarthritis of the hip and knee joints who had undergone arthroplasty. Methods: Patients completed a web-based survey after watching 40 videos that demonstrated 10 functional tasks with increasing difficulty, prior to and at approximately 3-5 months following surgery. The Lower Extremity Functional Scale (LEFS) was used as the reference measure. Longitudinal convergent and known-groups validity as well as sensitivity to change were assessed. Results: The data of 120 patients (n=80, 67% female; mean age 67, SD 9 years; n=58, 48% with hip osteoarthritis and n=62, 52% with knee osteoarthritis) were examined.
