Congenital Heart Defects: Understanding Mixed Blood Flow Conditions#CongenitalHeartDefects #PediatricCardiology #MedicalEducation #NursingNotes #iprkashmishra

Understanding Common Congenital Heart Diseases and Their Development The human heart is one of the earliest and most vital organs to develop in an embryo, beginning to beat just four weeks after fertilization to supply nutrients and oxygen necessary for survival. Any disruptions during early cardiac development can result in congenital heart diseases (CHDs). This article explains the formation of the heart from the embryonic mesoderm, focusing on the lateral visceral layer, which forms the cardiovascular system. Initially, two groups of cells form a horseshoe-shaped cardiogenic field that eventually fuses into the primitive heart tube, which gives rise to key heart structures such as the atria, ventricles, and major arteries. The article highlights three common CHDs: tricuspid atresia, tetralogy of Fallot, and patent ductus arteriosus (PDA). Tricuspid atresia occurs when the tricuspid valve fails to develop properly, leading to underdeveloped right ventricles and abnormal blood flow that often requires early medical intervention. Tetralogy of Fallot combines four defects, including right ventricular outflow obstruction, ventricular hypertrophy, ventricular septal defect, and an overriding aorta, causing oxygen-poor blood to mix with oxygenated blood and leading to cyanosis. PDA results from the failure of the fetal ductus arteriosus to close after birth, causing abnormal blood flow from the aorta to the pulmonary artery and potential heart or lung complications. The article emphasizes early diagnosis and intervention to improve outcomes for patients with these congenital conditions.

#usability Evaluation of a Mixed Reality Platform in Pediatric Interventional Cardiology by Specialist Physicians: Mixed Methods Study Background: Immersive technologies such as virtual reality (VR), augmented reality (AR), and mixed reality (MR) are used in pediatric interventional cardiology (IC) to improve anatomical visualization, spatial understanding and support procedural precision. These technologies offer three-dimensional (3D) representations of complex cardiac structures, which can aid in pre-procedural planning, intraoperative navigation, and team communication. As these technologies gain traction in IC, understanding how medical specialists perceive their value is vital. Objective: This study assesses what IC specialist physicians consider valuable in immersive technologies by evaluating their experiences with CardioVision, a software designed for pediatric IC procedures. Methods: A mixed methods design was used, combining questionnaires and semi-structured interviews. Purposeful and snowballing sampling was used to recruit specialist physicians with experience in IC. The recruited specialist physicians were asked to submit IC cases for software testing. These were uploaded to CardioVision by the developer who also conducted the testing in person with the physicians at their designated clinical stations. A total of eight specialist physicians completed a pre-test questionnaire, used the software, and thereafter completed a post-test questionnaire, which included System #usability Scale (SUS) and Likert-scale questions on experience with the software. The interviews explored the specialist physicians’ experience with the software’s performance. Of the eight participants, six further participated in the interviews. The interviews were conducted by two of the authors. The qualitative data were analyzed using inductive content analysis, involving subjective interpretation of textual material from the interviews. Following the approach outlined by Elo and Kyngäs, the process comprised three phases: preparation, organization, and reporting. In the preparation and organization phases, one author organized and categorized the interview data in an Excel sheet. The two authors then reviewed the material, compared emerging codes and themes, and reached consensus on the final main theme and subthemes for reporting. Results: In total, eight questionnaires and six interviews were obtained from specialist physicians from Belgium, France, Germany and Italy. Quantitative data from mean SUS scores showed that users felt confident using the system (mean = 4.25). In use of the software, the highest positive feedback was for visualization of structures, where all users (100%) agreed. Overall, most participants (75%) rated the system positively for decision-making support and 63% for complex case preparation. A minority (12%) disagreed with its effectiveness in preparing complex cases, indicating mixed opinions in this area. In the interviews, emerging themes aligned with quantitative data to show that participants valued the software’s ease of use, high-quality visualization, and decision-making support. Additional benefits included use in medical education, interprofessional collaboration, and patient communication. Developer–clinician collaboration was emphasized as vital for effective integration into routine practice. Conclusions: Immersive technologies offer clinical value, when aligned with user needs and integrated through collaborative development.

The Validation and Accuracy of Wearable Heart Rate Trackers in Children With Heart Disease: Prospective Cohort Study Background: Wearables are increasingly used in pediatric cardiology for heart rate (HR) monitoring due to advantages over traditional heart rate monitoring, such as prolonged monitoring time, increased patient comfort and ease of use. However, their validation in this population is limited. Objective: The objective is to assess HR accuracy and validity from two wearables, the Corsano CardioWatch bracelet and Hexoskin shirt, in children attending the pediatric cardiology outpatient clinic, exploring factors that influence accuracy, the Hexoskin shirt's arrhythmia detection efficacy, and patient satisfaction. Methods: Children indicated for 24h-Holter monitoring were equipped with a 24h-Holter ECG (gold standard), together with both wearables. HR accuracy was defined as percentage of HRs within 10% of Holter values and agreement was assessed using Bland-Altman analysis. Subgroup analyses were conducted based on body mass index (BMI), age and time of wearing, among other factors. The association between accelerometry (expressed in gravitational units, g) and HR accuracy was analyzed to assess the impact of bodily movement on measurement accuracy. A blinded pediatric cardiologist analyzed Hexoskin shirt data for rhythm classification. Patient satisfaction was measured using a 5-point Likert-scale questionnaire. Results: Thirty-one participants (mean age 13.2±3.6 years; 45% female) and thirty-six (mean age 13.3±3.9) participants were included for the CardioWatch and Hexoskin measurements respectively. Mean accuracy was 84.8% (±8.7%) for the CardioWatch and 87.4% (±11.0%) for the Hexoskin shirt. Hexoskin shirt accuracy was notably higher in the first 12 hours (94.9±7.4%) compared to the latter 12 (80.0±16.7%, P
