Researchers Identify How Lecanemab Activates Microglia to Clear Alzheimer’s Plaques Researchers at the Vlaams Instituut voor Biotechnologie (VIB) and KU Leuven have uncovered the mechanism by which the Alzheimer’s drug lecanemab, marketed as Leqembi, works in the brain. The study reveals that the drug’s effectiveness relies on a specific part of the antibody called the Fc fragment. This fragment acts as a trigger for microglia, the brain’s immune cells, prompting them to clear toxic amyloid plaques associated with Alzheimer's disease. Previous uncertainty about the drug’s mechanism has now been resolved, demonstrating that without the Fc fragment, microglia remain inactive and plaque clearance does not occur. Using a mouse model containing human microglial cells, researchers observed that activated microglia remove plaques through processes such as phagocytosis and lysosomal activity, with gene activity patterns, including the expression of SPP1, correlating with effective plaque clearance. The findings suggest new strategies for Alzheimer’s treatments, potentially allowing future therapies to activate microglia directly, without relying on antibodies. The study was published in Nature Neuroscience and funded by several organizations including the European Research Council and the Alzheimer’s Association.
