Virologist Finds New Antiviral Strategies in Virus Study Beyond both being viruses, HIV-1 and SARS-CoV-2 don't seem to have a lot in common. HIV-1 is a retrovirus that integrates with its host's DNA for life and can be passed down from mother to child, while SARS-CoV-2 is contagious but temporary and cannot be inherited. And while antivirals can repress the symptoms caused by each infection, effective vaccines exist only for one. But for Rockefeller virologist Theodora Hatziioannou, the viruses have one very important thing in common: They both cause deadly pandemics. Hatziioannou is well known for her pioneering research on the molecular interactions between HIV-1 and its host. Among other insights, she discovered that these interactions were species-specific, which explained why it had been nearly impossible to create an animal model for HIV-1, the human-specific virus-and led her to find a way around it. Researchers have since used her HIV-1 animal model to not only observe the course of infection but also control and manipulate it, as well as to evaluate the effectiveness of HIV-1-specific therapeutics and prevention strategies. Underutilization of Flu Antivirals | Influenza (Flu) - CDC Although the U.S. flu season is winding down, flu activity remains elevated and people are continuing to get sick. Prescription antiviral drugs that can treat flu in children, adults, and pregnant women are available, but those drugs are under-used. Two recent studies found that even patients with flu who are at higher risk of developing severe flu complications are not always being prescribed antiviral drugs as recommended. CDC recommends that people who are very sick with flu (for example, hospitalized) be treated with influenza antiviral medications as soon as possible. CDC likewise recommends that people with suspected or confirmed flu who are not hospitalized but are at higher risk of getting very sick from flu also receive influenza antiviral treatment as soon as possible. The recommendations also allow otherwise healthy people, including children, who have the flu to receive antiviral treatment to shorten the duration of their illness and decrease the risk for some complications. The Data A study in Pediatrics found that, from December 2016 through March 2020, only about half of children hospitalized with flu were treated with antivirals. Ordinary differential equation models of SARS-CoV-2 replication dynamics and antiviral drug efficacies In this chapter, we examine general viral replication dynamics within the human body without addressing immune response or treatment dynamics. Conceptually, virus replication models focus on three different scales: cell level, tissue level, and population level. For the tissue scale level, within-host models of SARS-CoV-2 focus on the respiratory tract and target cells expressing ACE24,24. Main target sites are the lower respiratory tract (LRT), upper respiratory tract (URT), and nasopharyngeal cells in the nose25,26. Only one study simulates infections in extrapulmonary organs27. For example, Raach et al. developed a tissue-dependent simple TCL model that models various cell populations in the respiratory tract and characterizes different infection dynamics there, leading to the conclusion that ciliated epithelial cells are the main target of SARS-CoV-2 infection based on the concentration of ACE2 receptors24. These tissue-based models indicate how anatomical structures are able to influence parameter estimation and viral kinetics, revealing that whole body dynamics cannot be reliably inferred from single tissue models. Antiviral use low among older COVID outpatients, despite higher risk for severe disease A University of Calgary–led research team finds that only 7% of children infected with Shiga toxin–producing Escherichia coli (STEC) developed hemolytic uremic syndrome (HUS) during a 395-case outbreak at 11 daycare centers in Alberta, Canada, perhaps partly due to daily lab monitoring for kidney injury. The researchers investigated the outbreak among children and adults related to contaminated food served at the centers in August 2023, publishing the findings yesterday in JAMA Network Open. An estimated 15% to 20% of children infected with STEC develop HUS, more than half of whom require dialysis. “Alberta, Canada, has one of the highest STEC infection rates in North America,” the authors wrote. “Accordingly, the province has developed expertise in diagnostics, public health responses, and care pathways.” Substantial use of health care resources In total, 326 primary and 33 secondary infections occurred among 288 children and 71 adults. Of the 359 infections, 18.4% caused no symptoms. There were 508 and 395 emergency department and STEC clinic visits, respectively, among the 285 children with outcome data.
