Gut microbiome clues may help spot Parkinson's disease in its earliest phase A new study suggests that shifts in the gut microbiome may occur before Parkinson’s disease is clinically diagnosed, raising the possibility of earlier detection in genetically at-risk individuals and even in seemingly healthy individuals. Image Credit: Kateryna Kon / Shutterstock In a recent study published in the journal Nature Medicine, researchers investigated whether gut microbiome data could be used to identify individuals who may be closer to Parkinson’s disease (PD) onset or progression, rather than directly predict future PD risk. The study used fecal metagenomics data from a cohort of 464 individuals, including diagnosed PD patients (n = 271), non-manifesting GBA1 variant carriers (GBA-NMC; n = 43), and healthy controls (HC; n = 150) to identify PD-specific microbiome alterations that may reflect a premanifest or prodromal PD state. Study findings revealed that a large component of the gut microbiome, representing slightly over 25% of microbiome abundance, in at-risk individuals (GBA-NMC cohort) exists in an intermediate state between health and disease. This microbial signature was also observed alongside similar PD-related microbiome alterations across three independent international cohorts, supporting the reproducibility of the PD-associated microbiome pattern across populations. While it is against the law to capture wild parrots, buying and selling captive-bred individuals remains legal. Along with the high demand for parrots as pets, the difficulty in distinguishing captive-bred from wild individuals is one reason the intelligent birds remain among the most-trafficked animals in the world, according to the Convention on International Trade in Endangered Species. Tools exist for identifying illegally trafficked parrots, but they require specialized training, are time-consuming, and can be stressful for the animals, says Steven Janssen, a mobile veterinarian for the World Parrot Trust. As a result, officials mostly use a subjective process to determine which parrots warrant investigation for potential trafficking. “You base your decision on behavior. Is this an animal that is timid and starts screaming when you approach?” Janssen says. “Even for people who are into birds and know birds, this is difficult.” Now, researchers are refining a new method that could help law enforcement quickly and easily distinguish wild from captive-bred individuals. Habitual coffee intake shapes the gut microbiome and modifies host physiology and cognition This prospective study had three phases each evaluating cognition, stress, physical health, mood, immune function, gut microbiome, dietary intake, and metabolite composition (see Fig. 1 for design). Firstly, we compared non-coffee drinkers (NCD, n = 31) with coffee drinkers (CD, n = 31), then the CD group abstained from coffee for 14 days. Following this washout period, participants reintroduced either decaffeinated (n = 15) or caffeinated coffee (n = 16) for 21 days (Fig. 1A). Participants in the NCD group did not participate in any subsequent phases of the study. Overall, more females participated, and most participants were born per vaginum (Table 1). At baseline, CD participants consumed more caffeine daily than NCDs, and NCDs gave up more caffeine before the baseline visit (Table 1, Supplementary Data 1), though the level given up was substantially lower than what would be considered addictive. Alcohol consumption, education years, Childhood Trauma Questionnaire (CTQ) sub-scores, and predicted IQ (measured using the National Adult Reading Test (NART)) did not differ between groups (Table 1). Microbiome signature of Parkinson's disease in healthy and genetically at-risk individuals A total of 540 participants were included in the full analysis set for clinical data. Participants’ characteristics are shown in Table 1. Participants with PD (n = 314) included both carriers (n = 128) and non-carriers (n = 186) of GBA1 variants (Appendix A). The individuals without PD included healthy controls non-carriers of GBA1 variants (n = 175, HC) and non-manifesting GBA1 variant carriers (n = 51, GBA-NMC). When age and sex of GBA-NMC were compared to the other groups, no differences were detected. Participants with PD were significantly older than HC (P= 0.006) and more frequently males (P = 0.028). More than half of the HC participants were partners of people with PD, to mitigate the effect of diet or other lifestyle-associated variables on gut microbiome composition. Clinical profile of GBA-NMC To identify possible clinical elements that could stratify GBA-NMC for their risk of developing PD, we compared the severity of motor and non-motor symptoms between HC and GBA-NMC using a wide range of clinical scales and questionnaires (Table 1). Gut microbiome can reveal risk of Parkinson's, scientists say Changes to microbes that live in the gut can identify people at greater risk of Parkinson’s disease long before symptoms develop, according to work that also raises hopes for new therapies. Researchers discovered signature changes in the gut microbiome that are more pronounced in people with a genetic risk for Parkinson’s and even more stark in those diagnosed with the disease. The signature could help doctors spot patients at risk of Parkinson’s years before they display clear symptoms and suggests that healthier diets and treatments that reshape the microbiome might prevent or delay the disease. Prof Anthony Schapira, the head of clinical and movement neurosciences at University College London and lead investigator on the study, said it was the first time a microbial signature in Parkinson’s patients had been seen in people with a genetic susceptibility but had yet to develop symptoms.
